虚拟现实技术在老年脑卒中后平衡功能及步行能力训练中的应用效果 被引量:2

The Effect of Virtual Reality Technology in the Training of Balance Function and Walking Ability after Stroke in the Elderly

摘要 目的探讨虚拟现实技术应用于老年脑卒中后平衡功能及步行能力训练中的效果.方法选取2019年1月—2020年9月入住上海市金山区众仁老年护理医院的36例老年脑卒中后平衡功能及步行能力障碍患者,随机分为对照组与试验组,各18例.两组均采用常规康复治疗,对照组进行普通平衡杠内步行训练,试验组进行虚拟现实技术步行训练.比较两组干预前后的平衡功能、下肢运动功能、步行能力.结果干预前,比较两组患者的Berg平衡量表(Berg balance scale,BBS)评分、下肢Fugl-Meyer运动功能量表(Fugl-Meyer assessment,FMA)评分、步态评估量表(Tinetti gait analysis,TGA)评分和"起立-行走"计时测试(time"up and go"test,TUGT)结果,差异无统计学意义(P>0.05);干预4、8周后,两组的BBS、FMA、TGA评分均高于干预前,且试验组评分均高于对照组,组间差异有统计学意义(P<0.05);两组的TUGT用时均短于干预前,且试验组用时短于对照组,组间差异有统计学意义(P<0.05).结论虚拟现实技术应用于老年脑卒中平衡功能及步行能力训练中的疗效良好,可改善患者的平衡功能及步行能力. Objective To explore the effect of virtual reality technology in the training of balance function and walking ability after stroke in the elderly.Methods A total of 36 elderly patients with post-stroke balance and walking ability disorders who were admitted to Zhongren Elderly Nursing Hospital in Jinshan District,Shanghai from January 2019 to September 2020 were randomly divided into a control group and an experimental group,with 18 cases in each group.Both groups received conventional rehabilitation treatment.The control group received walking training in a normal balance bar,and the experimental group received walking training with virtual reality technology.The balance function,lower limb motor function,and walking ability of the two groups before and after intervention were compared.Results Before the intervention,the Berg balance scale(BBS)score,lower limbs Fugl-Meyer assessment(FMA)score,Tinetti gait analysis(TGA)score and the"up and go"test(TUGT)results of the two groups were compared,and the differences were not statistically significant(P>0.05);after 4 and 8 weeks of intervention,the BBS,FMA,and TGA scores of the two groups were higher than before the intervention,and the scores of the experimental group were higher than those of the control group,the differences between the groups were statistically significant(P<0.05);the time used for TUGT in both groups was shorter than before the intervention,and the time used in the experimental group was shorter than that in the control group,the differences between the groups were statistically significant(P<0.05).Conclusion The effect of virtual reality technology in the balance function and walking ability training of elderly stroke is good,and it can improve the balance function and walking ability of patients.
