摘要
目的观察中西医结合综合管理对慢性心力衰竭患者心功能、明尼苏达生活质量调查表评分、中医证候积分、血浆NTProBNP水平的影响。方法将120例心功能Ⅱ~Ⅲ级的慢性心力衰竭出院患者,随机分为管理组和对照组,管理组按照方案给予中西医结合综合管理,对照组给予常规门诊随访;观察24周,主要观察管理前后的中医症候评分、心功能分级、NT-proBNP、生活质量调查表评分(LiHFe调查表评分)、再次入院次数以及平均住院费用等指标。结果经过24周综合管理和临床观察,管理组在心功能疗效、明尼苏达生活质量调查表评分、中医症候疗效、血浆NT-ProBNP水平和再次入院次数方面均优于对照组(P<0.05),2组再次入院平均费用、死亡率无明显差异(P>0.05)。结论中西医结合综合管理可以显著改善慢性心力衰竭患者心脏功能,降低血浆NT-proBNP水平,改善明尼苏达生活质量调查表评分和中医症候积分,减少再次入院次数,节约总医药费用。
Objective To observe the effectiveness of integrated management of traditional Chinese and western medicine on cardiac function,Minnesota quality of life score,TCM syndrome score and plasma NT-Pro BNP level in patients with chronic heart failure.Methods 120 cases of cardiac functionⅡ~Ⅲlevel of patients with chronic heart failure after their discharge from the hospital were randomly divided into management group and the control group.The management group was given integrated management of integrated traditional Chinese and western medicine according to the plan,and the control group received routine outpatient follow-up.The observation period was 24 weeks.TCM syndrome score,cardiac function score,NT-Pro BNP,life quality questionnaire score,number of readmission and average hospitalization cost were observed before and after management.Results After 24 weeks of integrated management and clinical observation,the management group was superior to the control group in terms of cardiac function efficacy,Minnesota quality of life questionnaire score,TCM syndrome efficacy,plasma NT-pro BNP level and number of realizations(P<0.05).There was no significant difference in average cost and mortality between the two groups(P>0.05).Conclusion The integrated management of traditional Chinese and western medicine can significantly improve the heart function of patients with chronic heart failure,reduce the level of plasma NT-pro BNP,improve the score of Minnesota quality of life questionnaire and TCM syndrome score,reduce the number of readmission,and save the total medical costs.
