摘要
目的:探究依达拉奉联合阿加曲班治疗对脑梗死患者血流动力学及神经功能缺损程度的影响分析。方法:收集脑梗死患者共160例,随机分为观察组(80例)和对照组(80例)。对照组给予阿加曲班进行治疗,观察组给予依达拉奉联合阿加曲班进行治疗。比较两组患者的治疗前后神经功能缺损程度(CSS)评分、美国国立卫生院卒中评分(NIHSS)、治疗前后相关炎症因子、内皮素1(ET^-1)、血管内皮细胞生长因子(VEGF)、同型半胱氨酸(Hcy)、神经元特异性烯醇化酶(NSE)水平、大脑的血流动力学。结果:治疗后,观察组的CSS评分及NIHSS评分分别为(22.65±4.24)分及(5.25±2.15)分,显著低于对照组的【(25.14±4.43)分,(7.16±3.47)分】。治疗后观察组白介素-6(IL-6)、、肿瘤坏死因子-α(TNF-α)、ET^-1、Hcy、NSE水平分别为(119.64±16.27)ng·L^-1、(13.85±6.74)ng·L^-1、(52.28±13.12)ng·L^-1、(9.58±3.12)μmol·L^-1、(13.64±6.25)μg·L^-1,均显著低于对照组的【(181.45±17.63)ng·L^-1、(27.54±8.93)ng·L^-1、(75.35±11.46)ng·L^-1、(15.16±4.22)μmol·L^-1、(17.19±7.47)μg·L^-1】,VEGF水平为(831.63±152.34)ng·L^-1,显著高于对照组的【(745.26±154.65)ng·L^-1】。治疗后观察组Vs、Vm水平分别为(62.66±10.69)cm·s^-1、(86.23±16.86)cm·s^-1,均显著高于对照组的【(58.86±11.06)cm·s^-1、(81.45±12.58)cm·s^-1】,PI水平为(0.73±0.11)显著低于对照组的(0.83±0.38)。观察组的总有效率为96.25%,显著高于对照组总有效率83.75%。结论:依达拉奉联合阿加曲班可有效的降低脑梗死患者的炎症反应,提高感染预防效果率,减少大脑神经受损并提高了神经功能及日常生活活动能力,且安全性较高,值得临床应用和推广。
Objective To explore the effect on hemodynamics and degree of neurological deficits in patients with cerebral infarction by edaravone combined with argatroban.Methods A total of 160 patients with cerebral infarctionwere collected and randomLy divided into the observation group(80 cases)and the control group(80 cases).The control group was given argatroban for treatment,while the observation group was given edaravone combined with argatroban for treatment.The neurological deficit degree scores of Degree of neurological impairment(CSS)and national institutes of health stroke score(NIHSS),related inflammatory factors,endothelin^-1(ET^-1),vascular endothelial growth factor(VEGF),homocysteine(Hcy),neuron-specific enolase(NSE)levels,cerebral hemodynamics were compared between the two groups.Results After treatment,the scores of CSS and NIHSS score in the observation group were(22.65±4.24)and(5.25±2.15)respectively,significantly lower than those of the control group[(25.14±4.43),(7.16±3.47)].After treatment,the levels of interleukin-6(IL-6),tumor Necrosis Factor-α(TNF-α),ET^-1,Hcy and NSE in the observation group were(119.64±16.27)ng·L^-1,(13.85±6.74)ng·L^-1,(52.28±13.12)ng·L^-1,(9.58±3.12)μmol·L^-1,(13.64±6.25)μg·L^-1,respectively.They were significantly lower than those of the control group[(181.45±17.63)ng·L^-1,(27.54±8.93)ng·L^-1,(75.35±11.46)ng·L^-1,(15.16±4.22)μmol·L^-1,(17.19±7.47)μg·L^-1],VEGF level was(831.63±152.34)ng·L^-1,significantly higher than those of the control group[(745.26±154.65)ng·L^-1]After treatment,the levels of Vs and Vm in the observation group were(62.66±10.69)cm·s^-1 and(86.23±16.86)cm·s^-1 respectively,which were significantly higher than those in the control group[(58.86±11.06)cm·s^-1,(81.45±12.58)cm·s^-1],PI level was(0.73±0.11)significantly lower than those in the control group[(0.83±0.38)].The total effective rate in the observation group was 96.25%,significantly higher than that in the control group 83.75%.Conclusion Edaravone combined with argatroban can effectively reduce the inflammatory reaction of cerebral infarction patients,improve the infection prevention effect rate,reduce cerebral nerve damage,and improve the nerve function and daily living activities.It is safe and worthy of clinical application and promotion.
