摘要
目的:研究补肾疏肝健脾方对反复体外受精-胚胎移植(IVF-ET)失败患者子宫内膜容受性及妊娠结局的影响。方法:将2019.1~2020.4我院收治的92例反复IVF-ET失败患者纳入研究,按照随机数表法均分为观察组(补肾疏肝健脾方治疗)和对照组(常规治疗)各46例,比较两组患者临床疗效、妊娠结局,比较两组患者治疗前及治疗3个月经周期后中医症状积分、子宫内膜容受性[子宫内膜厚度、阻力指数(RI)、搏动指数(PI)]、疾病相关因子[白血病抑制因子(LIF)、整合素αVβ3],统计两组用药期间不良反应发生情况。结果:两组总有效率比较无显著差异(P>0.05)。治疗3个月经周期后,观察组中医症状积分、RI、PI水平明显低于治疗前及同期对照组,子宫内膜厚度、疾病相关因子(LIF、整合素αVβ3)水平明显高于治疗前及同期对照组(P<0.05)。观察组临床妊娠例数明显高于对照组,妊娠失败例数明显低于对照组(P<0.05)。两组不良反应发生情况比较无显著差异(P>0.05)。结论:补肾疏肝健脾方能够改善反复IVF-ET失败患者临床症状及子宫容受性,提升临床妊娠率。
Objective:To study the effect of Bushen Shugan Jianpi Recipe on endometrial receptivity and pregnancy outcomes for patients with repeated in vitro fertilization-embryo transfer(IVF-ET)failure.Methods:92 patients with repeated IVF-ET failure admitted to our hospital from January 2019 to April 2020 were included in the study.According to the random number table method,the patients were divided into an observation group(Bushen Shugan Jianpi Recipe)and a controlled group(routine treatment),with 46 cases in each group.The clinical efficacy and pregnancy outcomes were compared between the two groups,and the TCM symptoms scores and endometrial receptivity[endometrial thickness,resistance index(RI),pulsatility index(PI)]and disease-related factors[leukemia inhibitory factor(LIF),integrinαVβ3]were compared between the two groups before treatment and after 3 menstrual cycles of treatment.The occurrence of adverse reactions during medication were counted in the two groups.Results:There was no significant difference in the total effective rate between the two groups(P>0.05).After 3 menstrual cycles of treatment,the TCM symptoms scores and levels of RI and PI of the observation group were significantly lower than those before treatment and those of the controlled group during the same period while the endometrial thickness and levels of disease-related factors(LIF,integrinαVβ3)were significantly higher than those before treatment and those of the controlled group during the same period(P<0.05).The number of clinical pregnancy cases in the observation group was significantly higher than that in the controlled group while the number of pregnancy failure cases was significantly lower than that in the controlled group(P<0.05).There were no significant differences in the occurrence of adverse reactions between the two groups(P>0.05).Conclusion:Bushen Shugan Jianpi Recipe can improve the clinical symptoms and uterine receptivity,and increase the clinical pregnancy rate of patients with repeated IVF-ET failure.
