摘要
目的比较人工全髋或者半髋关节置换对股骨颈骨折老年病患者的疗效。方法选取2019年1月至12月本院收治的股骨颈骨折老年患者共82例为研究对象,依据随机数表法将其分成对照组(予以全髋关节置换)和观察组(予半髋关节置换),每组各41例,比较两组各项手术指标与术后1年的髋关节功能及并发症情况。结果观察组术中失血量及引流量少于对照组,手术时长短于对照组(P<0.05)。对照组术后1年的关节功能优良比例高于观察组,感染、压疮、假体松动及关节脱位等术后并发症发生率低于观察组(P<0.05)。结论予以股骨颈骨折老年患者半髋置换术有着手术耗时短和术中失血及引流量少等优点,而人工全髋置换可获取更理想的远期疗效,同时并发症较少,需要临床结合患者实际合理选取术式开展治疗。
Objective This paper aims to compare the efficacy of total hip arthroplasty or hemiarthroplasty in elderly patients with femoral neck fracture.Methods 82 elderly patients with femoral neck fracture in our hospital from January to December2019 were selected as the research objects.According to the random number table method,they were divided into the control group(treated with total hip replacement)and the observation group(treated with half hip replacement),41 cases in each group.The operation indexes,hip joint function and complications of the two groups were compared.Results The intraoperative blood loss and drainage volume of the observation group were less than those of the control group,and the operation time of the observation group was longer than that of the control group(P<0.05).The rate of excellent and good joint function in the control group was higher than that in the observation group,and the incidence of postoperative complications such as infection,pressure sore,prosthesis loosening and joint dislocation in the control group was lower than that in the observation group(P<0.05).Conclusion Hemiarthroplasty for elderly patients with femoral neck fracture has the advantages of short operation time,less intraoperative blood loss and drainage,while total hip arthroplasty can obtain more ideal long-term curative effect with less complications,so it is necessary to reasonably select the operation mode for the treatment in combination with the actual situation of patients.
