摘要
目的分析全髋关节置换术(THA)与半髋关节置换术(HA)治疗老年股骨颈骨折的临床效果。方法选取我院2016年1~12月收治的240例老年髋关节置换术患者作为研究对象,根据治疗方式的不同分为THA组和HA组,各120例,比较两组的临床疗效。结果 THA组的手术时间长于HA组,术中出血量多于HA组,差异有统计学意义(P<0.05)。两组的住院时间比较,差异无统计学意义(P>0.05)。THA组的疼痛程度优于HA组,差异有统计学意义(P<0.05)。THA组的并发症发生率低于HA组,差异有统计学意义(P<0.05)。结论 THA和HA两种手术方式均具有良好的临床效果,实际应用中要结合患者的具体情况,对于身体条件较好、术后活动频繁或较年轻的患者,应尽量采取THA。
Objective To analyze the clinical effect of total hip arthroplasty （THA） and hemiarthroplasty （HA） in the treatment of senile femoral neck fractures.Methods 240 patients with senile femoral neck fractures in our hospital from January to December in 2016 were selected as research objects.According to the different treatment methods,they were divided into the THA group and the HA group,120 cases in each group.The clinical effect were compared between the tWO groups.Results The operation time of the THA group was longer than that of the HA group,and the blood loss of the THA group was higher than that of the HA group,with significant difference （P〈0.05）.There was no significant difference in hospitalization time between the two groups （P〉0.05）.The degree of pain in the THA group was better than that in the HA group,with significant difference （P〈0.05）.The incidence rate of complication in the THA group was lower than that in the HA group,with significant difference （P〈0.05）.Conclusion Both of two treatment methods of THA and HA have better clinical＇effect and its application should be based on the specific conditions of patients.THA should be applied to young patients or patients who have better physical condition and frequent post-operative activities.
