摘要
目的分析2015-2017年浙江省西南山区某县中老年HIV/AIDS病人感染特征及疫情增长原因。方法2017年12月,问卷调查该县所有2015-2017年HIV确证阳性且年龄≥40岁者一般特征、高危行为及检测史等,结合HIV系统进化树判断感染途径及时间地点。结果共调查109例,平均年龄(59.61±10.16)岁,男性75例(68.81%),其中80.00%(60/75)经商业性行为感染,女性34例(31.19%)。判定为2014年之前、2014、2015、2016年感染者,本地感染的分别为19/27、14/17、22/23、37/42,呈上升趋势(χMH^2=4.244,P=0.039);商业异性性传播及其二代传播病例占历年本地感染者的9/19、6/14、14/22、31/37,呈逐年上升趋势(χMH^2=9.860,P=0.002)。本地感染的46例嫖客中农民36例,小学及文盲34例和已婚有配偶的27例,60~69岁的26例,住址距暗娼点距离:M=4.8km(P25~P75:2.6~12.5km)。结论该县疫情上升主要由于本地商业异性性传播及其二代传播病例的增长幅度升高,当地暗娼点周边的低文化中老年嫖客是桥梁人群及重点干预对象,应考虑加强宣教并进一步动员检测。
Objective To analyze the epidemiological characteristics of HIV/AIDS infection among the people ≥40 years old reported from 2015 to 2017 in the southwestern of Zhejiang province. Methods A face-to-face questionnaire survey was conducted and blood samples were collected to analyze their characteristics, high-risk behaviors, history of HIV testing, infection route, time and place. Results A total of 109 participants were surveyed, including 75 males and 34 females aged 59.61±10.16 years. Among the males, 80%(60/75) were infected via commercial sex behavior. The proportions of local cases infected with HIV were 19/27, 14/17, 22/23, and 37/42 before 2013, in 2014, 2015, and 2016, showing a upward trend(χMH^2=4.244, P =0.039). The proportions of cases infected locally through commercial sexual behaviors and secondary transmission were 9/19, 6/14, 14/22, and 31/37(χMH^2=9.860, P =0.002). Among the men infected locally through commercial sex, there were(36/46) of farmers, 34/46 with education of primary school and below, 27/46 married, and 26/46 aged between 60-69 years. The distance between the men and the places of female sex workers was M=4.8 km(P25-P75: 2.6-12.5 km). Conclusion The proportion of cases infected locally through commercial sex and secondary transmission is increasing in recent years. The local men living around the entertainment establishments of female sex workers were the key group for AIDS prevention and control.
