摘要
目的对深圳地区艾滋病暴露后预防阻断现况和阻断效果进行分析,为开展艾滋病病毒(HIV)暴露前和暴露后预防阻断提供技术和政策依据。方法收集2019年1-6月间深圳市第三人民医院艾滋病预防阻断门诊所有就诊者信息,对人口学特征、就诊时间段、就诊原因、暴露时间、处理结果、复诊情况及不良事件情况进行分析。结果 2019年1-6月,本院艾滋病预防阻断门诊共接诊805人次病人,日均7.1人次就诊,其中初诊病人514例。就诊者以男性为主,就诊年龄集中在21~50岁性活跃年龄段。514例初诊者就诊原因包括73.0%(375人)因同性性行为就诊,15.0%(77人)因异性性行为就诊,6.2%(32人)因职业暴露就诊,5.8%(30人)因恐艾或咨询等其他原因就诊。93.2%(479人)初诊病人评估后需要且同意接受阻断用药;3个月复诊HIV抗体转阳率为0;复诊率较低(14天复诊率51.5%、28天复诊率58.4%、3个月复诊率17.8%)。结论医疗机构开展艾滋病预防阻断门诊是尽早发现HIV感染、降低HIV新发感染的有效途径,但接受预防阻断的咨询、阻断后随访和医学观察是难点和重点。
Objective To analyze the current status and effect of AIDS post-exposure prophylaxis in Shenzhen, and to provide basis for policy development of pre-and post-exposure prophylaxis. Methods We extracted the data from all patients visiting the AIDS clinic of Shenzhen Third People’s Hospital during Jan-Jun 2019, and analyzed the demographic characteristics, period for counseling, reasons for counseling, exposure duration, treatment, subsequent visits, and adverse events. Results During the study period, a total of 805 patients were admitted to the AIDS clinic, with an average of 7.1 visits per day, including 514 first-visit patients. The majority of the patients were male, with the age between 21-50 years old. Reasons for clinic visiting included high-risk homosexual behavior(73.0%), high-risk heterosexual behavior(15.0%), occupational exposure(6.2%), and other non-high-risk behaviors such as AIDS phobia or counseling(5.8%). 479(93.2%) first-visit patients agreed to receive post-exposure prophylaxis after assessment. The positive seroconversion rate of HIV antibody was 0% after 3 months of subsequent visits. However, the rate of return visit was relatively low(51.5% at 14 days;58.4% at 28 days;17.8% at 3 months). Conclusion The AIDS clinics in medical institutions can play an effective role in detecting HIV infection and reducing new HIV infections. However, it is difficult for the patients to receive counseling of PrEP and PEP knowledge, the follow-up visits after their prophylaxis, and medical observation.
