摘要
目的探究PDCA循环对神经外科昏迷患者围手术期压力性损伤发生率及预后的影响。方法选取本院2019年5月-2020年5月在本院神经外科治疗的昏迷患者150例为研究对象,采取随机数字表法将患者分为两组,对照组(75例)实施常规护理,研究组患者(75例)以PDCA循环管理法实施护理干预。统计并对比两组患者的压力性损伤预防效果、高危压力性损伤部位的皮肤情况以、患者家属的护理满意度、神经功能缺损评分及生活质量。结果研究组患者压力性损伤风险较对照组更低,差异具有统计学意义(P<0.05)。两组患者治疗后的高危部位皮肤评分较治疗前下降明显,且研究组下降幅度较对照组低,差异具有统计学意义(P<0.05)。干预后,试验组患者家属满意度(96.00%)明显较对照组高(82.67%),差异具有统计学意义(P<0.05)。研究组患者NIHSS评分低于对照组,而SF-36评分高于对照组,差异具有统计学意义(P<0.05)。结论PDCA循环管理法在神经外科昏迷患者中应用价值极高,能够明显降低患者的围手术期压力性损伤发生情况,改善其预后,提高病人家属满意度,可推广应用。
Objective To explore the effect of PDCA cycle on the incidence and prognosis of perioperative stress injury in neurosurgical coma patients.Methods 150 comatose patients treated in neurosurgery department of our hospital from May 2019 to may2020 were selected as the research objects.The patients were divided into two groups by random number table method.The control group(75 cases)was given routine nursing care,and the study group(75 cases)was given PDCA Cycle management method for nursing intervention.Prevention effect of stress injury,skin condition of injury parts,nursing satisfaction,neurological deficit score and quality of life between two groups of patients were statistically compared.Results The risk of stress injury in the study group was lower than that in the control group(P<0.05).After treatment,the skin score of high-risk injury parts in two groups decreased significantly compared with that before treatment,and the decrease rate of the study group was lower than that of the control group,the difference was statistically significant(P<0.05).After the intervention,the satisfaction degree of family members in the experimental group(96.00%)was significantly higher than that in the control group(82.67%),the difference was statistically significant(P<0.05).NIHSS score of the study group was lower than that of the control group,while SF-36 score was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion The application value of PDCA cycle management method in neurosurgical coma patients is very high.It can significantly reduce the incidence of perioperative stress injury,improve the prognosis and satisfaction degree of patients.
