摘要
目的:分析针灸配合加味二仙汤治疗脾肾气虚证慢性肾小球肾炎(CGN)的疗效。方法:选取82例在医院治疗的脾肾气虚证CGN患者作为研究对象,按照随机数字表法将其分为研究组(n=41例)与对照组(n=41例)。两组患者均给予常规基础治疗,在此基础上对照组患者给予加味二仙汤治疗,研究组患者则给予针灸配合加味二仙汤治疗,两组患者均治疗3个月。记录两组患者临床疗效,比较治疗前、治疗3个月后两组患者中医证候(腰脊酸痛、疲倦乏力、浮肿、食少纳呆)积分、24h尿蛋白定量、肾功能[血肌酐(Scr)、尿素氮(BUN)]及血清炎性因子[白介素6(IL-6)、肿瘤坏死因子(TNF-α)]水平变化情况。结果:研究组患者临床总有效率明显高于对照组(P<0.05)。治疗3个月后,两组患者腰脊酸痛、疲倦乏力、浮肿、食少纳呆等中医证候积分均低于治疗前,且研究组较同期对照组明显更低(P<0.05);两组患者24h尿蛋白定量较治疗前显著改善,且研究组优于对照组(P<0.05);两组患者Scr、BUN等肾功能指标及IL-6、TNF-α等炎性因子水平较治疗前明显降低,且研究组明显低于同期对照组(P<0.05)。结论:针灸配合加味二仙汤可改善脾肾气虚证CGN患者临床症状,降低24h尿蛋白定量,减轻炎性水平,恢复肾功能。
Objective:To analyze the efficacy of acupuncture and moxibustion combined with Jiawei Erxian decoction in the treatment of chronic glomerulonephritis(CGN)with spleen-kidney qi deficiency.Methods:82 cases of CGN patients with spleen-kidney qi deficiency treated in our hospital were selected as the research subjects,and they were divided into study group(n=41)and control group(n=41)according to the random number table method.Patients in the two groups were given conventional basic treatment,and patients in control group were given Jiawei Erxian decoction,and patients in study group were given acupuncture and moxibustion combined with Jiawei Erxian decoction,and the two groups were treated for 3 months.The clinical efficacy of the two groups was recorded,and the scores of TCM syndromes(lumbar spine pain,fatigue and weakness,edema,reduced appetite and anorexia),24 hurinary protein quantification,renal function serum creatinine(Scr),urea nitrogen(BUN)]and serum inflammatory factors interleukin 6(IL-6),tumor necrosis factor(TNF-α)]were compared between the two groups before treatment and after 3 months of treatment.Results:The total clinical effective rate in study group was significantly higher than that in control group(P<0.05).After 3 months of treatment,the scores of TCM syndromes such as lumbar spine pain,fatigue and weakness,edema and reduced appetite and anorexia in the two groups were lower than those in control group,and the scores in study group were significantly lower than those control group during the same period(P<0.05).The 24 hurinary protein quantification in the two groups was significantly improved compared with that before treatment,and the index in study group was better than that in control group(P<0.05).The levels of renal function indexes of Scr and BUN and inflammatory factors of IL-6 and TNF-αin the two groups were lower than those before treatment,and the levels in study group were significantly lower than those in control group during the same period(P<0.05).Conclusion:Acupuncture and moxibustion combined with Jiawei Erxian decoction can improve the clinical symptoms,reduce the 24 hurinary protein level,relieve the inflammation level,and restore renal function of CGN patients with spleen-kidney qi deficiency syndrome.
