Clinical Efficacy and Safety Analysis of Applying Suchan Yishen Decoction Combined with Invigorating Spleen and Invigorating Kidney Acupuncture in the Treatment of Chronic Nephritis with Proteinuria

摘要目的:探究苏蝉益肾汤结合健脾益肾针刺法治疗慢性肾炎蛋白尿临床疗效及安全性。方法:合计纳入103例慢性肾炎蛋白尿患者,均由我院2019年5月~2021年1月收治,以随机数字表法将患者分成两组,对照组52例患者采取西医治疗,观察组51例患者采取西医治疗+苏蝉益肾汤结合健脾益肾针刺法治疗,比较两组患者疗效及治疗前后中医症状(浮肿、腰脊酸痛、疲倦乏力、胃脘胀满、便溏等)积分变化、24h尿蛋白、肌酐及尿素水平、白介素-6(IL-6)及白介素17(IL-17)水平变化、T淋巴细胞亚群指标变化及不良反应。结果:较对照组(84.62%),观察组有效率(96.08%)更高,P<0.05;治疗前,两组患者中医症状(浮肿、腰脊酸痛、疲倦乏力、胃脘胀满、便溏等)积分、24h尿蛋白、肌酐及尿素水平、IL-6及IL-17、CD3^(+)、CD4^(+)、CD8^(+)水平比较,P>0.05,治疗后各组患者中医症状(浮肿、腰脊酸痛、疲倦乏力、胃脘胀满、便溏等)积分、24h尿蛋白、肌酐及尿素水平、IL-6及IL-17、CD3^(+)、CD4^(+)、CD8^(+)水平均改善,观察组患者治疗后中医症状(浮肿、腰脊酸痛、疲倦乏力、胃脘胀满、便溏等)积分、24h尿蛋白、肌酐及尿素水平、IL-6及IL-17、CD3^(+)、CD4^(+)、CD8^(+)水平优于对照组,P<0.05;两组不良反应率均低,P>0.05。结论:苏蝉益肾汤结合健脾益肾针刺法治疗慢性肾炎蛋白尿临床疗效佳,安全可靠,值得应用。 Objective:To explore the clinical efficacy and safety analysis of applying Suchan Yishen Decoction combined with invigorating spleen and invigorating kidney acupuncture in the treatment of chronic nephritis proteinuria.Methods:A total of 103 patients with chronic nephritis proteinuria were enrolled in our hospital from May 2019 to January 2021.The patients were divided into two groups by the random number table method.The controlled group(n=52)received western medicine treatment,and the observation group(n=51)received western medicine treatment+Suchan Yishen Decoction combined with spleen and kidney strengthening acupuncture.The efficacy,scores changes of TCM symptoms(edema,lumbar spine pain,fatigue,epigastric distention,loose stool,etc.),24 h urinary protein,creatinine and urea levels,interleukin-6(IL-6)and interleukin-17(IL-17)levels,changes of T lymphocyte subsets indexes and adverse reactions in 2 groups were compared before and after treatment.Results:Compared with the controlled group(84.62%),the effective rate of the observation group(96.08%)was higher,P<0.05.Before the treatment,the two groups of patients with symptoms of TCM(dropsy,weakness of lumbar spinal pain,fatigue,the diagnosis bilges full,loose stool,etc.),integral,24 h urine protein,creatinine and urea levels,IL-6 and IL-17,the level of CD3^(+),CD4^(+),CD8^(+),P>0.05,groups of patients after treatment of TCM symptoms(dropsy,weakness of lumbar spinal pain,fatigue,the diagnosis bilges full,loose stool,etc.)integral,24 h urine protein,creatinine and urea levels,IL-6 and IL-17,CD3^(+),CD4^(+),CD8^(+)levels are improving.After treatment,scores of TCM symptoms(edema,lumbar spine pain,fatigue,epigastric distention,loose stool,etc.),24 h urinary protein,creatinine and urea levels,IL-6 and IL-17,CD3^(+),CD4^(+),CD8^(+)levels in the observation group were better than those in the controlled group(P<0.05).The adverse reaction rate in both groups was low(P>0.05).Conclusion:Applying Suchan Yishen Decoction combined with invigorating spleen and invigorating kidney acupuncture in the treatment of chronic nephritis proteinuria has good clinical efficacy,safe and reliable,worthy of application.
