摘要
目的:探讨针灸治疗脑卒中后便秘的临床选穴规律。方法:运用计算机检索国内外医学数据库,建立针灸治疗脑卒中后便秘的临床研究文献数据库,运用数据挖掘技术,对选取的腧穴、经络等进行频次统计,对使用频次在10次及以上的腧穴进行聚类分析和关联分析。结果:纳入242篇文献,涉及85个腧穴,使用总频次934次,天枢穴应用频次最高,占151次。涉及13条经脉,足阳明胃经使用频率最高,占35.87%。关联规则分析得出7个有效群集:足三里-天枢、中脘-天枢、上巨虚-天枢、天枢-大肠俞、气海-天枢、关元-天枢、支沟-天枢。聚类分析得出4个有效群集:左外水道-左外归来-水道-归来、中脘-下脘-气海-关元、支沟-照海-丰隆、天枢-大肠俞-上巨虚-足三里。结论:针灸治疗脑卒中后便秘临床取穴以足阳明胃经和任脉为主,多采取腹部局部取穴,配合远端循经取穴,俞募配穴、合募配穴得到广泛运用。
Objective:To explore the clinical acupoint selection rule of acupuncture and moxibustion in the treatment of post-stroke constipation.Methods:The database of clinical research on acupuncture and moxibustion treatment of constipation after stroke was established by computer retrieval of medical databases at home and abroad.The frequency statistics of selected acupoints and meridians were conducted by using data mining technology.Cluster analysis and association analysis were conducted for acupoints with frequency of 10 times or more.Results:A total of 242 literatures were included,involving 85 acupoints,with a total frequency of 934 times.Tianshu acupoint was the highest,accounting for 151 times.Involving 11 meridians,the stomach meridian of Foot Yangming was the most frequently used,accounting for 35.87%.According to association rule analysis,seven effective clusters are obtained:Zusanli-Tianshu,Zhongwan-Tianshu,Shangjuxu-Tianshu,Tianshu-Dachangshu,Qihai-Tianshu,Guanyuan-Tianshu,Zhigou-Tianshu.Cluster analysis shows that there are four effective clusters:left outer Shuidao-left outer Guilai-ShuiDao-Guilai,Zhongwan-XiawanQihai-Guanyuan,Zhigou-Zhaohai-Fenglong,Tianshu-Dachangshu-Shangjuxu-Zusanli.Conclusion:Acupuncture and moxibustion treatment of post-stroke constipation is mainly based on Foot Yangming stomach meridian and Ren meridian.Local abdominal acupoints and distal meridians are widely used.
