摘要
目的研究益气健脾通便方治疗肺脾气虚型慢传输型便秘对患者体内胃饥饿素(Ghrelin)、肿瘤坏死因子-α(TNF-α)的变化。方法选取医院2018年10月—2020年5月收治的肺脾气虚型慢传输型便秘患者90例,采用随机数字法分为对照组(45例)和益气健脾通便组(45例),分别给予常规对症治疗和常规对症治疗联合益气健脾通便方治疗,均治疗2周。比较两组疗效、复发情况,记录两组治疗前、治疗2周后结肠传输试验结果及生长抑素(SS)、血管活性肠肽(VIP)、Ghrelin、TNF-α的差异。结果益气健脾通便组的总有效例数为43例(95.56%,43/45),高于对照组的37例(82.22%,37/45),差异有统计学意义(P<0.05)。两组治疗前结肠传输试验结果组间比较,差异无统计学意义(P>0.05)。益气健脾通便组治疗2周后左半结肠通过时间[(13.44±3.05)h]、右半结肠通过时间[(16.74±2.96)h]、直肠通过时间[(36.48±7.84)h]、全结肠通过时间[(71.05±9.47)h]短于对照组(P<0.05)。两组治疗前SS、VIP、Ghrelin、TNF-α比较,差异无统计学意义(P>0.05)。益气健脾通便组治疗2周后Ghrelin水平高于对照组,SS、VIP、TNF-α水平低于对照组(P<0.05)。随访1年发现,益气健脾通便组复发9例(20.00%,9/45),大幅度低于对照组的20例(44.44%,20/45),差异有统计学意义(P<0.05)。结论益气健脾通便方可调节肺脾气虚型慢传输型便秘患者Ghrelin、VIP、TNF-α、SS的表达水平,改善结肠传输功能,减少复发。
Objective To study the effect of Yiqi Jianpi Tongbian Recipe(益气健脾通便方)on ghrelin and tumor necrosis factor-α(TNF-α)in patients with slow transit constipation with Qi deficiency of lung and spleen.Methods Ninety patients with slow transit constipation of Qi deficiency of lung and spleen type from October 2018 to May 2020.The patients were divided into control group(45 cases)and Yiqi Jianpi Tongbian Recipe group(45 cases)by the random digital method.The results of colonic transmission test before and after 2 weeks of treatment and the differences of somatostatin(SS),vasoactive intestinal peptide(VIP),ghrelin and TNF-αof two groups were recorded and compared.Results A total of 43 cases were effective in the Yiqi Jianpi Tongbian Recipe group.The incidence was 95.56%(43/45),which was significantly higher than that in the control group(82.22%,37/45)(P<0.05).Before treatment,there was no significant difference of colonic transmission test between the two groups(P>0.05).The left colon transit time[(13.44±3.05)h],the right colon transit time[(16.74±2.96)h],the rectum transit time[(36.48±7.84)h]and the whole colon transit time[(71.05±9.47)h]were shorter than those of the control group(P<0.05).There was no significant difference of SS,VIP,Ghrelin or TNF-αbefore treatment in two groups(P>0.05).The ghrelin level of Yiqi Jianpi Tongbian Recipe group was higher than that of control group after 2 weeks treatment,and the levels of SS,VIP and TNF-αwere lower than those of the control group(P<0.05).After 1 year of follow-up,there were 9 cases of recurrence in Yiqi Jianpi Tongbian Recipe group and the recurrence rate was 20.00%(9/45),which was significantly lower than that in the control group(44.44%,20/45)(P<0.05).Conclusion Yiqi Jianpi Tongbian Recipe can regulate ghrelin,VIP and TNF-αin patients with slow transit constipation of Qi deficiency of lung and spleen type.The expression level of SS can improve the colonic transmission function and reduce the recurrence.
