摘要
目的探讨新生儿末梢第一滴血和第二滴血血糖值与实验室生化血浆血糖之间的差异,为新生儿病房床旁血糖监测方法提供循证证据。方法纳入2017年9月—2018年1月收治的新生儿161例,于检验科工作人员静脉抽血后30 s内,采集患儿末梢第一滴与第二滴血,使用便携式快速血糖仪测量血糖值。使用配对t检验比较末梢第一滴血血糖值、末梢第二滴血血糖值与静脉血浆血糖值间的差异。使用克拉克误差删格法分析末梢血血糖间的差异。结果末梢第一滴血的床旁血糖监测值为(4.69±1.24)mmol/L,第二滴血的床旁血糖监测值为(4.73±1.29)mmol/L,差异无统计学意义(P>0.05)。末梢第一滴血和第二滴血均高于血浆血糖值(4.33±1.52)mmol/L,差异有统计学意义(t1=6.469,t2=7.225,P<0.001)。床旁第一滴血血糖监测值与血浆血糖值差异在20%内的百分比为99.38%,差异不具有临床意义。结论监测新生儿床旁血糖值时,建议采用第一滴血作为检验标本,既可避免血液的浪费,又可节省临床护理操作时间。
Objective To explore the differences of neonatal venous plasma,the first and second drops of capillary blood glucose values.To provide evidence-based suggestions for clinical practice of blood glucose monitor in the neonatal ward.Methods 161 cases of neonates admitted from September 2017 to January 2018 were included.We monitored the values of the first and second drops of capillary blood in 30 seconds after venous blood draw.The T-test and EGA was used for statistical analysis.Results The value of capillary blood glucose of the first drop was(4.69±1.24)mmol/L.The value of capillary blood glucose of the second drop was(4.73±1.29)mmol/L.There were no significant differences between the two groups(P>0.05).Conclusion It was recommended that the first drop of blood could be used as the sample of neonatal peripheral blood glucose monitoring,it not only avoid the waste of blood,but also save clinical operation time.
