目的:对227例儿童原发性中枢神经系统肿瘤(primary pediatric Central Nervous System Tumors,ppCNST)进行电镜超微形态观察,分析其超微病理改变特征,为临床准确诊断ppCNST提供超微病理学依据。方法:回顾性分析了2019年6月至2024年6月...目的:对227例儿童原发性中枢神经系统肿瘤(primary pediatric Central Nervous System Tumors,ppCNST)进行电镜超微形态观察,分析其超微病理改变特征,为临床准确诊断ppCNST提供超微病理学依据。方法:回顾性分析了2019年6月至2024年6月收集的227例18岁以下ppCNST病例的年龄和性别的分布情况与肿瘤的发生部位、组织学类型和分级以及超微病理学改变。结果:227例患儿的平均年龄为10.3岁,男女比例为1.52∶1,均为颅内肿瘤(100%,227/227)。颅内肿瘤中,相对于幕下肿瘤(22.9%,52/227)而言,以幕上(包括松果体-四叠体区)肿瘤(77.1%,175/227)为主。本组病例中,经超微病理学辅助诊断患儿227例,其中最常见的6种肿瘤分别为:颅咽管瘤(65.6%,149/227),脊索瘤(17.2%,36/227),垂体神经内分泌肿瘤(11.5%,26/227),髓母细胞瘤(4.4%,10/227),生殖细胞瘤(1.3%,3/227),星形细胞瘤(1.3%,3/227)。结论:儿童原发性中枢神经系统肿瘤的发病具有其特殊性,结合临床症状、影像学检查、组织病理学以及超微病理学对患儿进行综合诊断有利于提高诊断率,降低误诊率,改善患儿生存质量。对于疑难病例,影像学改变非特异性或者免疫组织化学假阴性的患儿,电镜检查具有独特优势。展开更多
