摘要
目的:探讨前置医嘱审核对儿童住院患者不合理医嘱应用。方法:选择我院2017年5月及2018年5月的住院儿童患者医嘱进行研究。数据包括2017年5月儿童住院患者医嘱共1 329条为对照组,2018年5月儿童住院患者医嘱共1 272条为观察组。对照组采取常规处方点评的方法,观察组则采取前置医嘱审核的方法。实施前置医嘱审核半年后比较两组不合理医嘱发生率,并分析原因。结果 :观察组审核率为100.00%,明显高于对照组的25.36%。观察组不合理医嘱发生率为0.94%,明显低于对照组的2.26%,差异有统计学意义(P <0.05);不合理医嘱原因分析结果显示,观察组在剂量、用药频率、溶媒、用药途径及重复用药方面不合理医嘱发生率明显低于对照组,观察组不合理医嘱集中在相互作用、适应症及品种选择上。结论 :前置医嘱审核可明显降低儿童住院患者不合理医嘱的发生率,减少剂量、用药频率、溶媒、用药途径及重复用药方面不合理医嘱的发生。
Objective:To explore the application of pre-review of medical orders in unreasonable medical orders of inpatients in children.Methods:The medical orders of inpatients in children in our hospital in May 2017 and May 2018 were selected for the study.The data included 1 329 medical orders of inpatients in May 2017 as a control group and 1 272 medical orders of inpatients in May 2018 as an observation group.The medical orders in the control group were reviewed by the routine prescription review method,and the medical orders in the observation group were reviewed by the pre-review method.The incidence of unreasonable medical orders half a year after the implementation of the pre-review was compared between the two groups,and the reasons were analyzed.Results:The review rate in the observation group was 100.00%,which was significantly higher than 25.36%in the control group.The incidence of unreasonable medical orders in the observation group was 0.94%,which was significantly lower than 2.26%in the control group,and the difference was statistically significant(P<0.05).The results of the analysis on the reasons of unreasonable medical orders indicated that the incidence of unreasonable medical orders in dose,drug use frequency,solvent,drug use route and repeated drug use in the observation group was significantly lower than that in the control group.The unreasonable medical orders in the observation group focused on interactions,indications and variety selection.Conclusion:Pre-review of medical orders can significantly reduce the incidence of unreasonable medical orders of inpatients in children,reduce the incidence of unreasonable medical orders in dose,drug use frequency,solvent,drug use route and repeated drug use.
