摘要
目的观察柴胡疏肝散加减治疗冠心病合并焦虑症的疗效。方法将符合纳入标准的80例冠心病稳定型心绞痛伴焦虑症患者按照患者意愿分为40例治疗组和40例对照组,对照组给予单硝酸异山梨酯40 mg,每日1次,瑞舒伐他汀钙片5 mg,每日1次,阿司匹林肠溶片100 mg,1次/d,艾司唑仑片1 mg治疗,治疗组在对照组西医常规治疗基础上加柴胡舒肝散加减,水煎服,300 mL,早晚分服治疗,2组疗程均为4周。比较2组患者治疗前后的西雅图心绞痛量表得分,中医的症候得分,6 min步行距离,汉密尔顿焦虑量表得分。结果经4周治疗后,治疗组汉密尔顿焦虑量表疗效(90%)优于对照组(80%),2组比较差异有统计学意义(P <0.05)。治疗组西雅图心绞痛量表得分,中医的症候得分及6 min步行距离均优于对照组,且差异均具有统计学意义(P <0.05)。结论柴胡疏肝散具有疏肝理气,活血化瘀通络的功效,能有效改善心肌缺血缺氧状况、增加冠脉血流量、降低胆固醇、抑制血管平滑肌收缩,有效减轻冠心病合并焦虑症症状,提高患者生活质量。
Objective Observation of the curative effect of Chaihu Shugan powder in treating coronary heart disease with anxiety disorder.Methods Eighty patients with stable angina pectoris with anxiety disorder were divided into two groups according to their wishes.The control group was treated with isosorbide mononitrate 40 mg/times,once a day,and Risuvastatin calcium tablets 5 mg/times,were given to the patients in the control group,and the patients in the control group were treated with isosorbide mononitrate once a day.Once a day,aspirin enteric coated tablets 100 mg/times,once a day,esazolam tablets 1 mg treatment,the treatment group was given western medicine on the basis of routine treatment plus Chaihu Shugan powder,water decoction,300 mL,the morning and evening divided into treatment.The course of treatment in both groups was 4 weeks.To compare the scores of Seattle angina scale before and after treatment between the two groups and the symptoms of traditional Chinese medicine.Waiting score,6 min walking distance,Hamilton anxiety scale score.Results After four weeks treatment,the curative effect of Hamilton anxiety scale in the treatment group(90%)was better than that in the control group(80%)(P<0.05).The scores of Seattle angina scale,TCM symptom score and 6 min walking distance in the treatment group were better than those in the control group,and the difference was statistically significant(P<0.05).Conclusion Chaihu Shugan powder has the effect of soothing liver and regulating qi,activating blood circulation to remove blood stasis and clearing collaterals,which can effectively improve myocardial ischemia and hypoxia,increase coronary bloodflow,reduce cholesterol,inhibit vascular smooth muscle contraction,and effectively relieve symptoms in patients with coronary heart disease complicated with anxiety disorder.Improve the quality of life of patients.
