摘要
目的探讨早期康复护理在乳腺癌根治术后患者中的应用效果。方法选取2017年2月至2019年2月间陕西省渭南市中心医院收治的行乳腺癌根治术的78例乳腺癌患者,采用随机数表法将其分为对照组与研究组,每组39例。对照组患者术后切口愈合后遵医嘱进行肢体活动,无具体的训练时间要求与监管机制,研究组患者术后1d且生命体征稳定的情况下接受早期康复护理干预。术后随访3个月,比较两组患者上肢功能(患肢肩关节水平外展角度、向上外展角度和肩胛骨上提距离)和术后并发症情况,并通过生活质量评价量表比较两组患者术前与术后3个月时生活质量的变化。结果术后3个月时,研究组患者患肢肩关节水平外展角度、向上外展角度和肩胛骨上提距离均高于对照组患者,差异均有统计学意义(均P <0. 05)。研究组患者术后并发症的发生率为5. 1%,低于对照组患者的23. 1%,差异有统计学意义(P <0. 05)。术前两组患者生活质量量表中情绪功能、社会功能、躯体功能、家庭功能和物质需求评分比较,差异无统计学意义(P> 0. 05),术后3个月时,研究组患者生活质量量表中情绪功能、社会功能、躯体功能和家庭功能评分均高于对照组患者,差异均有统计学意义(均P <0. 05),两组患者物质需求比较,差异无统计学意义(P> 0. 05)。结论早期康复护理能够促进乳腺癌根治术后患者上肢功能的恢复,降低相关并发症发生率,提高生活质量,值得推广。
Objective To explore the efficacy of early rehabilitation nursing in patients with breast cancer after radical mastectomy.Methods A total of 78 patients who underwent radical mastectomy at Weinan Central Hospital from February 2017 to February 2019 were selected and divided into a research group and a control group by random number table with 39 patients in each group.Patients with stable vital signs in the study group received early rehabilitation nursing intervention at 1 d after the operation.Patients in the control group performed limb exercises according to the doctor’s instructions after the wound healing,without specific training time requirements and supervision mechanism.After 3 months of follow-up,the upper limb function(horizontal abduction angle of shoulder joint,upward abduction angle,scapula elevation distance)and complications were compared between the two groups.The changes in quality of life beforethe surgery and at 3 months after the surgery were compared by Short Form 36 questionnaire(SF-36).Results The horizontal abduction angle,upward abduction angle and scapula elevation distance of affected shoulder joint were better in the study group than in the control group at 3 months after the operation(all P<0.05).The incidence of postoperative complications was 5.1%in the study group,which was lower than 23.1%of the control group(P<0.05).Before the surgery,there was no significant difference in the scores of emotional function,social function,physical function,family function and material demand between the two groups(all P<0.05).At 3 months after operation,the scores of emotional function,social function,physical function and family function were higher in the study group than in the control group,(all P<0.05)but there was no significant difference in material demand between the two groups(P>0.05).Conclusion Early rehabilitation nursing can promote the recovery of upper limb function,reduce the probability of related complications and improve the quality of life after radical mastectomy,which is worthy of promotion.
