血清同型半胱氨酸水平与糖尿病患者颈动脉内中膜增厚的关系

Relationship between serum homocysteine level and carotid intima-media thickening in diabetic patients

摘要 目的探讨糖尿病患者血清同型半胱氨酸(Hcy)水平与颈动脉内中膜增厚的关系,为临床防治提供参考。方法选取2016年2月—2018年2月深圳市保健委员会办公室接诊的80例糖尿病患者,根据患者颈动脉内中膜厚度不同分为增厚组42例和未增厚组38例。选取同期于深圳市保健委员会办公室40例接受健康体检者为对照组。分别比较三组人员年龄、性别、体质量指数(BMI)、血脂、血糖、Hcy以及糖化血红蛋白水平,并作多因素logistic回归分析。结果增厚组BMI为(28.49±3.49)kg/m2,明显高于未增厚组的(24.32±3.17)kg/m2和对照组的(22.91±2.21)kg/m2,且未增厚组又明显高于对照组(均P<0.05)。增厚组甘油三酯、低密度脂蛋白胆固醇、空腹血糖、Hcy、糖化血红蛋白均明显高于对照组,且高密度脂蛋白胆固醇明显低于对照组(P<0.05);未增厚组空腹血糖、Hcy、糖化血红蛋白均明显高于对照组,且高密度脂蛋白胆固醇明显低于对照组(P<0.05)。增厚组甘油三酯高于未增厚组,差异有统计学意义(P<0.05)。经logistic回归分析得出BMI、甘油三酯、低密度脂蛋白胆固醇、Hcy均是影响糖尿病患者颈动脉内中膜增厚的独立危险因素,而高密度脂蛋白胆固醇是糖尿病患者颈动脉内中膜增厚的保护因素(均P<0.05)。结论影响糖尿病患者颈动脉内中膜增厚的因素包括BMI、甘油三酯、低密度脂蛋白胆固醇、Hcy、糖化血红蛋白、高密度脂蛋白胆固醇。 Objective To investigate the relationship between serum homocysteine homocysteine(Hcy)level and carotid intima-media thickening(IMT) in diabetic patients. Methods Totally 80 cases of diabetic patients in Shenzhen Health Committee Office from February 2016 to February 2018 were enrolled and divided into thickening group(n=42) and no thickening group(n=38) according to carotid intima-media thickness. A total of 40 cases of healthy persons at the same period were acted as control group. The indexes such as age, gender, body mass index(BMI), blood lipid, blood glucose,Hcy and Hb A1 clevels were compared among the above three groups, and multivariate logistic regression was analyzed. Results The BMI in thickening group was(28.49±3.49) kg/m2, which was significantly higher than in the no thickening group [(24.32 ± 3.17) kg/m2] and the control group[(22.91±2.21) kg/m2], and which in the no thickening group was significantly higher than that in the control group. Compared to the control group, these indexes such as triacylglycerol, low density lipoprotein cholesterol(LDL-C), fasting blood glucose and glycosylated hemoglobin in thickening group were significantly higher, while high density lipoprotein cholesterol was significantly lower(all P<0.05). The no thickening group of fasting blood glucose and glycosylated hemoglobin were significantly higher than the control group, while the high density lipoprotein cholesterol was significantly lower than the control group(all P<0.05). Logistic regression analysis showed that BMI, triacylglycerol, LDLC were all independent risk factors for carotid artery thickening in diabetic patients, while high density lipoprotein cholesterol was the protective factor(all P<0.05). Conclusion The factors affecting carotid IMT in diabetic patients include BMI, triacylglycerol, LDL-C, glycosylated hemoglobin and high density lipoprotein cholesterol.
