摘要
目的比较芪苈强心胶囊联合卡维地洛与硝普钠治疗射血分数正常心力衰竭(HFNEF)的临床疗效。方法选取河北以岭医院2017年7—12月收治的HFNEF患者120例,按照病床号码不同分为对照组与试验组,各60例。对照组予以芪苈强心胶囊联合硝普钠治疗,试验组予以芪苈强心胶囊联合卡维地洛治疗。两组患者均连续治疗4周。比较两组患者临床疗效,治疗前后心功能指标〔左心室收缩末期内径(LVESD)、左心室舒张末期内径(LVEDD)、左心室射血分数〕、血浆N末端脑钠肽前体(NT-proBNP),复发率及病死率,并观察两组患者不良反应发生情况。结果两组治疗总有效率比较,差异无统计学意义(P>0.05)。治疗前后两组LVESD、LVEDD、左心室射血分数比较,差异无统计学意义(P>0.05)。治疗前两组NT-proBNP水平比较,差异无统计学意义(P>0.05);治疗后试验组NT-proBNP低于对照组(P<0.05)。两组复发率、病死率比较,差异无统计学意义(P>0.05)。两组均未发生严重不良反应。结论芪苈强心胶囊联合卡维地洛与硝普钠治疗HFNEF的临床疗效相当,均可有效改善患者心功能,缓解临床症状,减少复发,且安全性较高。
Objective To compare the clinical effect of Qiliqiangxin capsule combined with carvedilol and sodium nitroprusside for heart failure with normal ejection fraction(HFNEF).Methods A total of 120 cases of patients with HFNEF were selected from July to December 2017 in Hebei Yiling Hospital,which were divided into control group and test group according to the bed number,60 cases in each group.The control group was treated with Qiliqiangxin capsule combined with sodium nitroprusside,the test group was treated with Qiliqiangxin capsule combined with carvedilol.The two groups were treated for 4 weeks.The clinical effect,cardiac function index(LVESD,LVEDD,left ventricular ejection fraction),NT-proBNP before and after treatment,recurrence rate and fatality rate were compared between the two groups,and the incidence of adverse reactions were observed.Results There was no significant difference in the total effective rate between the two groups(P>0.05).Before and after treatment,there was no significant differences in LVESD,LVEDD,left ventricular ejection fraction between the two groups(P>0.05).Before treatment,there was no significant difference in NT-proBNP level between the two groups(P>0.05).After treatment,the NT-proBNP of test group was lower than control group(P<0.05).There was no significant differences in the recurrence rate and fatality rate of the two groups(P>0.05).No serious adverse reactions occurred in both groups.Conclusion The clinical efficacy of Qiliqiangxin capsule combined with carvedilol and sodium nitroprusside for HFNEF is the same as that of sodium nitroprusside,all of them can effectively improve cardiac function,alleviate clinical symptoms,reduce recurrence,and with high safety.
