摘要
目的研究钠钾镁钙葡萄糖注射液在老年患者经尿道前列腺电切术(TURP)补液中的应用效果。方法选取青岛市海慈医疗集团2013年2—10月行TURP患者60例,按照随机数字表法分为葡萄糖注射液组(试验组)和乳酸钠林格注射液组(对照组),各30例。测量并比较两组患者术前(T1)、术后30min(T_2)、术毕(T_3)的平均动脉压(MAP)、血氧饱和度(SpO_2)、心率(HR)、中心静脉压(CVP)、血糖水平及电解质(Na^+、K^+、Cl^-、Mg^(2+)、Ca^(2+))水平。结果两组患者T_1、T2、T3时相MAP、SpO_2、HR、CVP比较,差异无统计学意义(P>0.05);两组患者T_2、T_3时相MAP均低于T1时相,HR均高于T1时相,差异有统计学意义(P<0.05)。两组患者T_1、T_2时相的血糖水平比较,差异无统计学意义(P>0.05);两组患者各时相Na^+、K^+、Cl^-、Ca^(2+)水平比较,差异无统计学意义(P>0.05)。试验组T3时相血糖水平高于T_1时相,对照组T_2、T_3时相Mg^(2+)水平低于T1时相,差异有统计学意义(P<0.05);试验组T_3时相血糖、Mg^(2+)水平高于对照组,差异有统计学意义(P<0.05)。结论钠钾镁钙葡萄糖注射液的电解质配比及渗透浓度更接近于正常机体细胞外液水平,对于行TURP的老年患者,术中静脉滴注钠钾镁钙葡萄糖注射液更有利于维持电解质和代谢平衡。
Objective To investigate the effect of sodium potassium magnesium calcium and glucose injection in patients undergoing transurethral resection of prostate. Methods A total of 60 patients underwent transurethral resection of the prostate were selected from February to October of 2013 in Qingdao Hiser Medical Group,which were divided into the sodium potassium magnesium calcium and glucose injection group( experimental group) and sodium lactate ringer’ s injection group( control group),30 cases in each group. MAP,SpO2,HR,CVP,blood glucose and Na+,K+,Cl-,Mg2+,Ca2+at time points of the beginng of surgery( T1),30 min after surgery( T2),the end of surgery( T3) were detected and compared. Results There were no differences of MAP,Sp O2,HR,CVP at time point of T1,T2,T3 between two groups( P > 0. 05); at time point of T2,T3,the MAP were lower than T1,HR were higher than T1,the differences were statistically significant( P >0. 05). There was no differences of the blood sugar at time point of T1,T2 between two groups( P > 0. 05),there was no differences of Na+,K+,Cl-,Ca2+at time point of T1,T2,T3 between two groups( P > 0. 05). The blood glucose of the experimental group at time point of T3 was higher than T1,the Mg2+of the control group at time point of T2,T3 were lower than T1,the differences were statistically significant( P < 0. 05); the blood glucose and Mg2+of the experimental group at time point of T3 were higher than control group,the differences were statistically significant( P < 0. 05). Conclusion The ratio of electrolyte and osmotic concentration of sodium potassium magnesium calcium and glucose injection are closer to the body’ s normal extracellular fluid level,it has more advatages of infusion sodium potassium magnesium calcium and glucose injection to maintain electrolyte balance and metabolism,especially for patients undergoing transurethral resection of prostate.
