摘要
目的:观察硬膜外阻滞麻醉前30min静脉预输注6%羟乙基淀粉对患者麻醉后低血压的预防效果及不良反应发生的影响。方法:选取硬膜外阻滞麻醉行腹部手术的患者80例,随机分为观察组与对照组,两组患者在麻醉前30min分别静脉输注乳酸钠林格注射液和6%羟乙基淀粉,观察两组麻醉前(T0)、麻醉后1min(T1)、5 min(T2)、10 min(T3)、15 min(T4)、20min(T5)时收缩压(SBP)、心率(HR)变化及不良反应发生情况。结果:两组患者T0、T1、T2、T5时SBP、HR差异比较无统计学意义(P>0.05);观察组T3、T4时SBP显著高于对照组,HR显著低于对照组(均P<0.05);观察组低出血压、恶心呕吐发生率及麻黄素使用率均显著低于对照组(P<0.05)。结论:硬膜外阻滞麻醉前30min静脉预输注6%羟乙基淀粉可有效预防患者麻醉期间低血压的发生,并可显著降低不良反应,有利于手术顺利进行。
Objective： To observe effects of hydroxyethyl starch infusion 30 min before undergoing anesthesia on pre- vention of hypotension and adverse reactions. Methods： 80 cases of patients schedule for abdominal surgery under epidural block anesthesia were randomly divided into observation group and control group. 30 min before anesthesia started, Sodium Lactate RingeICs and 6% hydroxyethyl starch were given to the observation group and control group respectively. Systolic blood pressure （SBP） and heart rate （HR） changes and adverse reactions were observed and compared at time points including： be- fore anesthesia started （TO）, 1 rain after anesthesia started （T1）, 5 rain （T2）, 10min （T3）, 15 rain （T4）, 20 min （TS） after anesthesia started. Results： No significant differences in SBP, HR at TO, T1, T2, T5 were observed between the two groups （P〈 0. 051. SBP of the observation group at 10 rain （T3） and 15 rain （T4） were significantly higher, but HR were lower than that of the control group （P both 〈0. 051. Incidence of hypotension, nausea and vomiting and ephedrine usage were signifi- cantly lower than that of control group （P〈0. 051. Conclusion： Hydroxyethyl starch infusion 30 rain before undergoing anes- thesia on can prevent occurrence of hypotension and decrease incidence of adverse reactions, it is good for the procedure of surgery.
