摘要
目的:观察直丝弓和Tip-Edge Plus对安氏Ⅱ类Ⅰ分类牙周病正畸治疗的疗效。方法:选择2014年1月-2017年1月在我院口腔正畸科就诊的安氏Ⅱ类Ⅰ分类牙周炎患者135例。对照组(n=66例)接受直丝弓正畸治疗,观察组(n=69例)采用Tip-Edge Plus正畸治疗。记录两组正畸术前和术后6个月的视觉模拟评分法(VAS)、探诊深度(PD)、临床附着丧失(CAL)、前牙唇侧角化龈宽度(KGW)、出血指数(BI)、牙周袋指数(PPI)、牙齿菌斑指数(DPI)、前牙和全牙Bolton指数、牙列排齐时间及满意度调查。结果:正畸术前,两组患者的VAS、KGW、PD、CAL、PPI、DPI、BI、前牙和全牙Bolton指数差异无统计学意义(P>0. 05),正畸术后6个月,与对照组比,观察组的KGW增高(P<0. 05),PD和CAL、PPI、DPI、BI、前牙和全牙Bolton指数明显降低(P<0. 05)。与对照组比,观察组患者牙齐排列时间较短(P<0. 05),患者及口腔医师的满意度评分均增高(P<0. 05)。结论:与直丝弓比,采用Tip-Edge Plus正畸可明显缩短安氏Ⅱ类Ⅰ分类牙周病正畸患者的牙列排齐时间,改善病理性参数。
Objective: To compare the effect of orthodontic treatment of straight wire arch and Tip-Edge Plus on angle class Ⅱperiodontitis.Methods: A total of 135 cases angle class Ⅱperiodontitis patients from January 2016 to January 2017 were divided into the control group(n=66) who received straight wire arch orthodontics and the observation group(n=69) who received Tip-Edge Plus orthodontics.The visual analogue scales(VAS), probing depth(PD), clinical attachment loss(CAL), keratinized gingival width(KGW), bleeding index(BI), periodontal pocket index(PPI), dental plaque index(DPI), the front and full tooth Bolton indexwere recorded and compared before and 6 months after orthodontic surgery. A survey of dentition alignment time and satisfaction were also recorded.Results: Before orthodontic surgery, there had no significant differences in the VAS, KGW, PD, CAL,PPI, DPI, BI, the front and full tooth Bolton index between the two groups(P>0. 05). After orthodontic treatment, compared with the control group, the KGW had increased(P<0. 05), the PD and CAL, PPI, DPI, BI, the front and full tooth Bolton index had decreased(P<0. 05) in the observation group, and the dentition alignment time had decreased, while the satisfaction scores of the patients and the dentists had increased(P<0. 05).Conclusion: Compared with the straight wire arch,the Tip-Edge Plus orthodontics can significantly shorten the dentition alignment time of Angle class Ⅱperiodontitis and improve the pathological parameters.
