血清类固醇受体共激活子联合传统炎症指标对肺炎支原体肺炎患儿反复呼吸道感染的预测价值

Predictive value of serum steroid receptor coactivators combined with traditional inflammatory indicators for recurrent respiratory tract infection in children with Mycoplasma pneumoniae pneumonia

摘要 目的分析血清类固醇受体共激活子(SRC)2、SRC3联合传统炎症指标对肺炎支原体肺炎患儿反复呼吸道感染(RRTI)的预测价值。方法选取2024年1月至6月在广西壮族自治区南溪山医院住院的303例肺炎支原体肺炎患儿,根据入院时是否诊断为RRTI分为RRTI组和对照组,检测血清SRC2、SRC3、C反应蛋白(CRP)、免疫球蛋白(Ig)G、IgA、IgM、白细胞介素(IL)-6、肿瘤坏死因子-α(TNF-α)水平。分析RRTI的影响因素,并通过受试者工作特征(ROC)曲线评估血清SRC2、SRC3等对RRTI的预测价值。结果与对照组比较,RRTI组血清SRC2、SRC3、IgA、IgM、IgG水平较低,CRP、IL-6、TNF-α水平较高(P<0.05)。logistic回归分析显示,CRP、IL-6、TNF-α是患儿发生RRTI的危险因素,IgG、SRC2、SRC3是患儿发生RRTI的保护因素(P<0.05);ROC曲线显示,血清CRP、IgG、IL-6、TNF-α、SRC2、SRC3预测RRTI的曲线下面积(AUC)分别为0.710、0.706、0.707、0.610、0.733、0.669,所有指标联合预测RRTI的AUC为0.898(95%CI:0.858~0.929),高于单一指标(P<0.05)。结论SRC2、SRC3联合传统炎症指标对RRTI具有较高的预测价值,可为肺炎支原体肺炎患儿RRTI提供诊疗依据。 Objective To analyze the predictive value of serum steroid receptor coactivator(SRC)2 and SRC3 combined with traditional inflammatory indicators for recurrent respiratory tract infection(RRTI)in children with Mycoplasma pneumoniae pneumonia.Methods A total of 303 children with Mycoplasma pneumoniae pneumonia who were hospitalized in Nanxishan Hospital of Guangxi Zhuang Autonomous Region from January to June 2024 were selected and divided into the RRTI group and the control group according to whether they were diagnosed with RRTI at admission.The levels of serum SRC2,SRC3,C-reactive protein(CRP),immunoglobulin(Ig)G,IgA,IgM,interleukin(IL)-6,and tumor necrosis factor-α(TNF-α)were detected.The influencing factors of RRTI were analyzed,and the predictive value of serum SRC2 and SRC3 for RRTI was evaluated through the receiver operating characteristic(ROC)curve.Results Compared with the control group,the levels of serum SRC2,SRC3,IgA,IgM and IgG in the RRTI group were lower,and the levels of CRP,IL-6 and TNF-αwere higher(P<0.05).The logistic regression analysis showed that CRP,IL-6 and TNF-αwere risk factors for RRTI in children,and IgG,SRC2 and SRC3 were protective factors(P<0.05).ROC curve showed that the area under the curve(AUC)of serum CRP,IgG,IL-6,TNF-α,SRC2 and SRC3 in predicting RRTI were 0.710,0.706,0.707,0.610,0.733 and 0.669,respectively.The AUC of combined prediction of RRTI was 0.898(95%CI:0.858-0.929),which was higher than that of each single index(P<0.05).Conclusion The combination of SRC2 and SRC3 with traditional inflammatory indicators has a high predictive value for RRTI and can provide a basis for the diagnosis and treatment of RRTI in children with Mycoplasma pneumoniae pneumonia.
