黑素细胞移植联合特色中药治疗稳定期白癜风临床疗效及影响因素分析

Clinical Efficacy and Influencing Factors Analysis of Melanocyte Transplantation Combined with Sepcialized Traditional Chinese Medicine for Stable Vitiligo Treatment

摘要 目的回顾性分析黑素细胞移植联合复方驱虫斑鸠菊丸治疗白癜风的临床疗效及影响因素。方法对比分析24例行黑素细胞移植联合复方驱虫斑鸠菊丸治疗和24例单独行黑素细胞移植治疗的稳定期白癜风患者的临床疗效,并探讨疗效影响因素。结果移植组白癜风皮损治疗有效率为37.50%,联合治疗组治疗有效率为66.67%,差异有统计学意义(P<0.05)。移植组患者40~45 d出现色素再生,9个月新发色素基本稳定;联合治疗组患者20 d左右出现色素再生,6个月新发色素基本稳定。联合治疗组白癜风活动度评分(vitiligo disease activity score,VIDA)为-1的患者治疗有效率最高(90.91%),与VIDA评分为0和1的患者相比差异有统计学意义(P<0.05);皮损中存在色素岛的患者治疗有效率较无色素岛的高,差异有统计学意义(P<0.05);移植面积与治疗有效率间差异无统计学意义(P>0.05)。结论黑素细胞移植联合复方驱虫斑鸠菊丸治疗稳定期白癜风临床疗效显著优于单独黑素细胞移植,联合治疗可促进皮肤复色进程,提高复色率。 Objective The clinical efficacy and influencing factors of melanocyte transplantation combined with compound Vernonia anthelmintica pills in the treatment of vitiligo were analyzed retrospectively.Methods A comparative analysis was conducted on the clinical efficacy and influencing factors of 24 stable vitiligo patients treated with melanocyte transplantation combined with compound Vernonia anthelmintica pills and 24 patients treated with isolated melanocyte transplantation.Results The treatment efficacy rate of vitiligo lesions in the transplantation group was 37.50%,while the treatment efficacy rate in the combination treatment group was 66.67%,and the difference was statistically significant(P<0.05).Patients in the transplantation group developed pigmentation within 40-45 days,and the newly developed pigmentation remained stable after 9 months.Patients in the combination treatment group developed pigmentation around 20 days,and the newly developed pigmentation remained stable after 6 months.The patients with a vitiligo disease activity score(VIDA)of-1 in the combination treatment group had the highest efficacy rate(90.91%)compared to patients with VIDA scores of 0 and 1,and the differences were statistically significant(P<0.05).The efficacy rate of patients with pigment islands in their skin lesions was higher than that without pigment islands,and the difference was statistically significant(P<0.05).There was no statistically significant difference between the transplantation area and efficacy rate(P>0.05).Conclusion The clinical efficacy of melanocyte transplantation combined with compound Vernonia anthelmintica pills in the treatment of stable vitiligo is significantly better than that of melanocyte transplantation alone.The combination therapy can promote the process of skin repigmentation and improve the repigmentation rate.
