区域医共体的医养融合健康管理探索--基于府西医养中心案例

摘要本文以府西医养中心作为案例,通过对医养融合健康管理的分析与研究,探索其在区域医共体框架下,医养融合健康管理事业发展的潜力。首先,本文概述了医养融合的初步探索与实践路径,阐述了府西医养中心医养融合健康管理的主要做法,包括整合医疗资源,提升服务质量、创新服务模式,满足个性化需求、加强健康教育与预防保健等。随后论述了府西医养中心医养融合健康管理的实施成效与具体的影响,最后分析了区域医共体的医养融合健康管理目前面临的挑战,并提出了应对对策与建议,以期为地区医共体建设和医养融合健康管理提供理论指导和经验借鉴。 This article takes the Western Medicine Elderly Care Center of the Prefectural Government as a case.Through the analysis and research on the integrated health management of medical care and elderly care,it explores the potential for the development of the integrated health management of medical care and elderly care under the framework of the regional medical community.Firstly,this article Outlines the initial exploration and practical paths of the integration of medical care and elderly care,and elaborates on the main practices of medical and elderly care integration health management in the Western Medicine Elderly Care Center of the Prefectural Government,including integrating medical resources,improving service quality,innovating service models,meeting personalized needs,strengthening health education and preventive health care,etc.Subsequently,the implementation effectiveness and specific impacts of the medical and elderly care integration health management in the Western Medicine Elderly Care Center of the prefecture were discussed.Finally,the current challenges faced by the medical and elderly care integration health management of the regional medical community were analyzed,and countermeasures and suggestions were put forward,expected to provide theoretical guidance and experience reference for the construction of regional medical communities and the integrated health management of medical care and elderly care.
