摘要
患者女,48岁。头顶部皮下结节2年,切除后复发1年。既往有骨肉瘤病史。皮肤科检查:颅顶部头皮见一线状手术瘢痕,其下可触及一孤立的结节,边界清楚,质硬,活动较差。皮损组织病理:真皮深部至皮下脂肪见数团软骨样结构,肿瘤团块中央见散在坏死组织,较多细胞核异形及病理性核分裂象。诊断:骨肉瘤皮肤转移。检索近20年国内外文献,仅有12例病例报道。
A 48-year-old female patient presented with a subcutaneous nodule on the vertex of the scalp for two years,which then recurred within one year after surgical resection.She had a history of osteosarcoma.Dermatological examination revealed that a linear surgical scar on the cranial vertex scalp,with an underlying isolated,firm and well-demarcated nodule with a poor mobility.Histopathological analysis demonstrated several cartilaginous-like structures extending from the deep dermis to subcu-taneous fat.The central area of the lesion contained scattered necrotic tissues,plus significant nuclear atypia and pathological mitotic features.A diagnosis of cutaneous metastasis from osteosarcoma was made.A literature review of studies reported in Chi-nese or English over the past 20 years identified only 12 cases.
