摘要
目的:探究固本益肠补虚方对肠易激综合征(irritable bowel syndrome,IBS)模型病情的影响并分析其潜在的作用机制。方法:通过化学刺激和急慢性压力刺激建立腹泻型IBS(IBS with diarrhea,IBS-D)大鼠模型,将40只IBS-D大鼠模型随机分为4组:对照组、模型组(给予生理盐水)、固本益肠补虚方组(给予固本益肠补虚方)以及利福昔明组(给予50 mg/kg利福昔明),每组10只。评估IBS-D大鼠粪便Bristol分级、粪便含水量和腹部撤退反射(abdominal withdrawal reflex,AWR)评分。采集结肠组织,评价固本益肠补虚方对其病理及促炎因子变化、NF-κB通路变化的影响。结果:与对照组比较,模型组AWR评分下降,粪便Bristol分级评分和含水量升高,结肠促炎因子水平升高,结肠组织损伤程度严重(P<0.05)。与模型组比较,利福昔明组和固本益肠补虚方组AWR评分升高,粪便Bristol分级评分和含水量降低,结肠促炎因子水平下降,结肠组织受损有明显好转(P<0.05)。与模型组比较,利福昔明组和固本益肠补虚方组p-IκBα/IκBα明显降低,p-p65/p65明显升高(P<0.05)。结论:固本益肠补虚方可改善IBS模型的肠道炎症反应,其机制可能与抑制NF-κB信号通路活性有关。
Objective:To explore the therapeutic effect of Gubenyichang Buxu Decoction on irritable bowel syndrome(IBS)and explore its potential mechanism of action.Methods:A rat model of diarrhea-predominant IBS(IBS-D)was established using chemical irritation combined with acute and chronic stress stimulation.Forty IBS-D model rats were randomly divided into four groups(n=1o per group):control group,model group(given normal saline),rifaximin group(given 50 mg/kg rifaximin)and the traditional Chinese medicine group(given Gubenyichang Buxu Decoction).Fecal Bristol grade,fecal water content,and abdominal retraction reflex(AWR)in IBSD rats were assessed.Colon tissue was collected to evaluate histopathological changes,inflammatory cytokine levels,and the activity of the NF-κB signaling pathway.Results:Compared with the control group,the model group showed decreased AWR scores,increased Bristol stool scores and fecal water content,elevated levels of pro-inflammatory cytokines in the colon,and more severe histological damage(P<0.05).Compared with the model group,both the rifaximin group and the Gubenyichang Buxu group exhibited significantly increased AWR scores,reduced Bristol stool scores and fecal water content,decreased levels of colonic pro-inflammatory cytokines,and improved histopathological features(P<0.05).Furthermore,the expression of p-IκBα/IκBα was significantly reduced,and p-p65/p65 was significantly increased in both treatment groups compared to the model group(P<O.O5).Conclusion:Gubenyichang Buxu Decoction can effectively alleviate intestinal inflammation in IBS-D model rats,possibly by modulating the NF-κB signaling pathway.
