摘要
原发性开角型青光眼(POAG)是一种慢性、进行性不可逆的致盲眼病,其主要表现为眼压升高、视神经损害和视野缺损而房角保持开放状态。POAG发病隐匿,早期诊断困难,常无自觉症状,严重威胁和损害视神经及视觉通路。控制眼压是延缓病情发展的主流治疗手段。POAG归属于中医“青盲”“青风内障”等范畴。目前,临床尚缺乏针对POAG统一的中医治疗标准。“子午流注”理论揭示了人体功能、病理与时间之间的变化规律,并进一步推论POAG的发病规律与“子午流注”理论的关系,据此从POAG的发病时间规律和辨证论治角度提出从肝、脾、肾等角度对患者采取不同时间选取不同功效中药及不同针刺穴位和方法等治疗方法以达控制眼压,进而控制或延缓视神经的凋亡、萎缩的作用,从而为POAG的治疗提供有益的探索。
Primary open-angle glaucoma(POAG)is a chronic,progressive,and irreversible blinding eye disease characterized by elevated intraocular pressure,optic nerve damage,and visual field defects,while the anterior chamber angle remains open.POAG has an insidious onset,making early diagnosis difficult,and often presents with no noticeable symptoms,posing a serious threat to and damaging the optic nerve and visual pathways.Controlling intraocular pressure is the mainstream approach to slowing disease progression.Currently,there is a lack of a unified standard for Traditional Chinese Medicine(TCM)treatment for POAG.POAG falls under the categories of Qingmang and Qingfeng Neizhang in TCM.The Ziwu Liuzhu theory reveals the relationship between human physiological functions,pathology,and time,and further deduces the connection between the pathogenesis of POAG and the Ziwu Liuzhu theory.Based on this,the article proposes treating POAG from the perspectives of liver,spleen,and kidney,selecting different Chinese medicines with varying effects,acupuncture points,and methods at different times according to the disease's onset patterns and syndrome differentiation.This approach aims to control intraocular pressure,thereby preventing or delaying optic nerve apoptosis and atrophy,and provides valuable insights for the treatment of POAG.
