摘要
医疗服务价格调整涉及高度的利益调节和资源优化利用。然而,目前医疗服务价格动态调整缺乏对利益相关主体的综合考虑,各利益主体之间的动态调整协作机制尚未形成。鉴于此,本文基于利益相关者理论系统分析了公立医疗机构、患者、医保基金和药品耗材供应商四大内圈利益相关者的利益诉求,阐述了医保部门、卫生健康部门、市场监管部门和财政部门四大外圈利益相关者各自的作用与责任,并在此基础上,构建内外圈利益相关者的作用机制。其次,提出形成医疗服务价格动态调整内外圈合力,推动价格改革与医保支付方式、医药领域改革有效衔接两大路径,并利用江西省X市的实践案例说明路径的可行性。最后,提出完善成本核算制度保障、优化公立医疗机构薪酬制度以及增强舆论宣传等建议。
Adjustment of medical service prices involves a high degree of interest regulation and optimal utilization of resources.However,the current dynamic adjustment of medical service prices lacks comprehensive consideration of relevant stakeholders,and a collaborative mechanism for dynamic adjustment among various stakeholders has not yet been established.In view of this,based on the stakeholder theory,this paper systematically analyzes the interest demands of the four inner-circle stakeholders,namely public medical institutions,patients,medical insurance funds,and drug and consumable suppliers.It elaborates on the respective roles and responsibilities of the four outer-circle stakeholders,including the medical insurance department,the health department,the market supervision department,and the finance department.On this basis,it constructs a mechanism of action for the inner and outer-circle stakeholders.Secondly,this paper proposes two paths:forming a joint force within the inner and outer circle stakeholders for the dynamic adjustment of medical service prices,and promoting the effective connection between price reform,medical insurance payment methods,and pharmaceutical sector reforms.The feasibility of these paths is llustrated by a practical case from City X in Jiangxi Province.Finally,suggestions such as improving the cost accounting system,optimizing the salary system of public medical institutions,and enhancing public opinion publicity are put forward.
