摘要
《千金方》为唐代医家孙思邈的经典专著,孙氏重小儿,对小儿病的治疗有丰富的论述。通过整理分析书中针对小儿病的外治法发现,《千金方》构建了以理同法异为核心的理论体系。基于小儿的生理病理特点,提出以知为度、因期择法的动态化干预策略,以及圆机活法、药借物性的外治应用创新。具体而言,不同类型的外治法之间各具特点,膏摩法将药物与手法相结合,擅以外治内;药浴法渍形以为汗,使邪从外而解;涂敷法经皮肤渗透吸收、在表治表,又可刺激穴位、激发经气;粉身法剂型特殊,尤善止汗;灸法相对小儿效力较强,须三因制宜、先后有序,为中医儿科外治学的发展奠定了重要基础,对现代中医外治研究具有重要启示。
Qian Jin Fang is a classic of SUN Si-miao,a doctor of the Tang Dynasty,who emphasized pediatricsand had rich discussions on pediatric disease treatment.By analyzing the external treatments for pediatric diseases in thebook,it was found that the book established a theoretical framework centered on the principle of"same theory,differentmethods".Based on the physiological and pathological characteristics of children,SUN proposed dynamic intervention strategies such as"suspension after medicine starting an effect"and"method selection according to disease stage".There werealso innovative applications of external treatments,including"flexible therapeutic approaches"and"treatment by the medicine's properties".Specifically,different types of external treatment methods have their characteristics.Ointment massagetherapy,a combination of medicine and manipulation,specializes in the external treatments of internal diseases.Medicinebath therapy impregnates the form to sweat to remove the pathogen from the outside.For application therapy,medicine isabsorbed through the skin,and exterior manifestations are treated externally.Additionally,application therapy stimulates theacupuncture points and the meridian qi.Powdering therapy uses special medicaments and is suitable for arresting sweat.Moxibustion,relatively potent for children,shall be adjusted according to conditions and applied orderly.These contributions laid a crucial foundation for the development of pediatric external treatments in traditional Chinese medicine and remain instructive for modern research on external treatments.
