摘要
目的探讨基于叙事医学教育的平行病历在外科实习生带教中的应用效果。方法选取2024年2月于中山大学附属第七医院外科实习的50名医学本科生为研究对象,按照随机数字表法分为常规组及叙事组,每组25例。其中常规组剔除1名实习生,叙事组剔除4名实习生,最终常规组和叙事组各有24名和21名实习生完成全程研究。常规组按实习大纲要求带教4个月,叙事组在常规带教同时开展叙事医学基础理论教学并书写平行病历,采用专业量表评估比较两组实习生带教前后共情能力、职业认同感及医患沟通能力差异。结果带教后,叙事组共情能力评分、职业认同感评分及医患沟通能力评分均高于本组带教前,而常规组仅医患沟通能力评分高于本组带教前,叙事组各评分均高于常规组,差异有统计学意义(P<0.05)。结论平行病历书写可提高外科实习生共情能力、职业认同感及医患沟通能力,适合在医学本科实习带教中实践推广。
Objective To explore the application effect of parallel medical records based on narrative medical education in the teaching of surgical interns.Methods Fifty medical undergraduates who were doing their internship in the Department of Surgery of the Seventh Affiliated Hospital,Sun Yat-sen University in February 2024 were selected as the research objects,they were divided into the conventional group and the narrative group according to the random number table method,with 25 cases in each group.Among them,one intern was removed from the conventional group and four interns were removed from the narrative group.Eventually,24 interns from the conventional group and 21 interns from the narrative group completed the entire study.The conventional group was taught for 4 months in accordance with the requirements of the internship syllabus.The narrative group,while conducting regular teaching,carried out the basic theory teaching of narrative medicine and wrote parallel medical records.Professional scales were used to evaluate and compare the differences in empathy ability,professional identity and doctor-patient communication ability between the two groups of interns before and after teaching.Results After the teaching,the scores of empathy ability,professional identity and doctor-patient communication ability in the narrative group were all higher than those before the teaching,while in the conventional group,only the score of doctor-patient communication ability was higher than that before the teaching,all scores of the narrative group were higher than those of the conventional group,and the differences were statistically significant(P<0.05).Conclusion Parallel medical records writing can improve the empathy ability,professional identity and doctor-patient communication ability of surgical interns,and is suitable for practical promotion in the teaching of medical undergraduate internships.
