摘要
从多囊卵巢综合征(polycystic ovary syndrome,PCOS)的中西医治疗现状及研究进展出发,探讨中西医治疗PCOS的相关机制。西医学认为,遗传因素、下丘脑-垂体-卵巢轴调节功能异常、胰岛素抵抗与高胰岛素血症、慢性炎症反应等,均可能是PCOS的发病原因。中医学理论指出,PCOS主要与肾、肝、脾三脏功能失调及痰湿、血瘀等病理因素密切相关。肾主生殖,若肾气不足或肾阳亏虚,可致天癸失期,冲任不调;肝郁会使气机不畅,阻碍气血运行;脾虚则无法正常运化水湿,水湿凝聚成痰,痰湿与血瘀相互胶结,阻滞胞宫,进而出现闭经、不孕、月经失调等症状。
The relevant mechanism of integrative medicine in the treatment of PCOS was discussed from the current state of treating PCOS with integrative medicine and research progress.Western medicine holds that genetic factors,the disorder of hypothalamic-pituitary-ovarian axis,insulin resistance and hyperinsulinemia,and chronic inflammatory reaction are the causes for PCOS.TCM theory believes that PCOS is closely related to the dysfunction of kidney,liver and spleen,and the pathogenic factors including phlegm-dampness,blood stasis and others.Kidney governs reproduction,the insufficiency of renal Qi or the depletion of renal Yang leads to irregular menstruation,disharmony of Chong channel and conception channel;liver stagnation causes Qi movement disorder,obstructing the circulation of Qi and blood;spleen deficiency fails to transport water dampness,and it could condense into phlegm,phlegm-dampness mixing with blood stasis,obstructing the uterus,thereby manifesting the symptoms such as amenorrhea,infertility and menstrual disorder.
