摘要
目的:探究治疗分泌性中耳炎(OME)的中药配伍规律,为本病的临床用药及新药开发提供前期理论参考。方法:通过检索中国知网中药复方及中西医结合治疗OME的临床研究文献,建立数据库,通过古今医案云平台对纳入数据进行用药频次及性味归经频次统计,并进行中药复杂网络、关联规则、聚类分析。结果:纳入的258首处方中共162味中药,其中高频药物有甘草、菖蒲、茯苓、柴胡等,药性以平、温、寒为主,药味以辛、甘、苦多见,归肺、脾、肝经最多,功效以利水渗湿、疏肝解郁、祛风止痛为主。复杂网络分析得到由菖蒲、茯苓、甘草、柴胡、川芎、桔梗、陈皮、白术、泽泻9味中药组成的核心处方;关联规则分析得到8对治疗OME的常用药对,川芎-柴胡、桔梗-甘草、陈皮-茯苓、香附-柴胡、白术-茯苓、香附-菖蒲、香附-川芎、半夏-茯苓,其中“柴胡-川芎”支持度最高;聚类分析得到4个聚类,聚类1:甘草;聚类2:桔梗、茯苓、陈皮;聚类3:白术、泽泻;聚类4:菖蒲、柴胡、川芎、香附。聚类分析结果表明,各医家治疗OME时所用药物多为二陈汤、泽泻汤、通气散加减合方,遵循疏风宣肺以通调水道,补脾以运化水湿,利水渗湿、疏肝行气以调畅气机的治疗原则。结论:古今医家治疗OME常从肺、脾、肝三脏入手,以“解表宣肺,运脾化湿的同时疏肝解郁、行气通窍”为主要治疗原则,为临床上中药新药开发提供理论基础。
Objective:To explore the compatibility rules of Chinese herbs for treating otitis media with effusion(OME),and to provide theoretical references for clinical medication and new drug development for the disease in the early stage.Methods:A database was established by searching the clinical research literature on OME treated by Chinese herbal compound and integrated Chinese and Western medicine in CNKI.The included data were subjected to statistical analysis for medication frequency,properties,flavors and channel tropism throug the Ancient and Modern Medical Case Cloud Platform.The complex network,association rules and cluster analysis of Chinese herbs were also carried out.Results:162 Chinese herbs were included in 258 prescriptions,among which the high-frequency drugs were Gancao,Changpu,Fuling,Chaihu and so on.The properties of the herbs were mainly neutral,warm,and cold,and the flavors were predominantly pungent,sweet,and bitter.The meridians mainly involved were the lung,spleen,and liver meridians.The main effects were clearing damp and promoting diuresis,soothing the liver and relieving depression and alleviating pain to expel wind.Complex network analysis revealed a core prescription composed of 9 herbs including Changpu,Fuling,Gancao,Chaihu,Chuanxiong,Jiegeng,Chenpi,Baizhu and Zexie.Association rule analysis identified 8 sets of common drug pairs for OME,namely Chuanxiong-Chaihu,Jiegeng-Gancao,Chenpi-Fuling,Xiangfu-Chaihu,Baizhu-Fuling,Xiangfu-Changpu,Xiangfu-Chuanxiong and Banxia-Fuling,among which“Chaihu-Chuanxiong”had the highest support.4 clusters were obtained by cluster analysis.Cluster 1:Gancao;Cluster 2:Jiegeng,Fuling and Chenpi;Cluster 3:Baizhu and Zexie;Cluster 4:Changpu,Chaihu,Chuanxiong and Xiangfu.The results of cluster analysis showed that modified Erchen Decoction,Zexie Decoction and Tongqi Powder were the most commonly used formulas in the treatment of OME by different doctors,which followed the therapeutic principles of dispelling wind and ventilating lung to regulate the waterway,tonifying spleen to transport water-dampness,clearing damp and promoting diuresis and soothing liver and promoting Qi circulation to regulate Qi activity.Conclusion:Ancient and modern medical practitioners often treat OME from the lung,spleen,and liver,focusing on resolving exterior conditions and ventilating the lung,strengthening the spleen and resoving dampness,as well as dispersing stagnated liver Qi to relieve depression,promoting circulation of Qi and freeing orifices as the main treatment principles.This provides a theoretical basis for the development of new Chinese herbs in clinical practice.
