摘要
探究地屈孕酮联合宫颈环扎用于预防习惯性晚期流产与早产的治疗价值。回顾性分析黔南布依族苗族自治州人民医院2021年3月—2024年8月收治的64例宫颈机能不全患者的临床资料,根据治疗方案的不同将其分为A组(n=34)和B组(n=30),均接受宫颈环扎手术治疗。在此基础上,A组应用硝苯地平治疗,B组在A组基础上联合应用地屈孕酮治疗,比较两组患者的相关指标。结果显示,B组足月分娩率较A组更高,早产率较A组更低(P<0.05);两组胎儿流产率比较,无统计学差异(P>0.05)。两组胎儿/新生儿以及患者并发症总发生率比较,无统计学差异(P>0.05);但B组胎膜早破发生率较A组更低(P<0.05)。B组新生儿体质量水平以及1 min、5 min Apgar评分均较A组更高(P<0.05)。两组患者的不良反应发生率比较,无统计学差异(P>0.05)。研究发现,对宫颈机能不全患者应用宫颈环扎联合地屈孕酮治疗可提升其足月分娩率,降低流产、早产率及胎膜早破发生率,提高新生儿出生质量,且安全性良好。
To explore the therapeutic value of dydrogesterone combined with cervical cerclage in the prevention of habitual late abortion and premature birth.A retrospective analysis was conducted on the clinical data of 64 patients with cervical insufficiency admitted to Qiannan Buyi and Miao Autonomous Prefecture People’s Hospital from March 2021 to August 2024.According to different treatment regimens,the patients were divided into group A(n=34)and group B(n=30),both receiving cervical cerclage surgery.On this basis,group A was treated with nifedipine,while group B was additionally treated with dydrogesterone on the ba-sis of group A.Relevant indicators between the two groups were compared.The results showed that the term birth rate in group B was significantly higher than that in group A,and the premature birth rate was lower than that in group A(P<0.05).There was no statistically significant difference in the fetal abortion rate between the two groups(P>0.05).There was no significant difference in the total incidence of fetal/neonatal and maternal complications between the two groups(P>0.05),but the incidence of prema-ture rupture of memb ranes in group B was lower than that in group A(P<0.05).The neonatal body weight,1-minute and 5-minute Apgar scores in group B were all higher than those in group A(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).It is concluded that the application of cervical cerclage combined with dydrogesterone in patients with cervical insufficiency can increase the term birth rate,reduce the incidence of abortion,premature birth and premature rupture of membranes,improve the birth quality of newborns,and has good safety.
