摘要
目的:分析术前应用经会阴高频超声联合腔内超声评估肛周脓肿及瘘管形成的准确性。方法:选取我院2021年10月至2024年6月90例疑似肛周脓肿及瘘管形成患者为研究参与者,均行经会阴高频超声、腔内超声检查,以病理结果为金标准,评估经会阴高频超声、腔内超声以及二者联合诊断对肛周脓肿及瘘管形成的检出情况、诊断价值,评估经会阴高频超声、腔内超声联合检测在不同病理类型中的检出情况。结果:90例疑似肛周脓肿及瘘管形成患者中,病理检查出肛周脓肿及瘘管患者88例;经会阴高频超声检查出肛周脓肿及瘘管患者80例;腔内超声检查出肛周脓肿及瘘管患者79例;经会阴高频超声、腔内超声联合检查出肛周脓肿及瘘管患者86例。病理检查出Ⅰ型20例,Ⅱ型21例,Ⅲ型14例,Ⅳ型15例,Ⅴ型18例,无病灶2例;经会阴高频超声、腔内超声联合检查出Ⅰ型21例,Ⅱ型20例,Ⅲ型15例,Ⅳ型14例,Ⅴ型16例,无病灶4例。经会阴高频超声诊断肛周脓肿及瘘管形成的敏感度、特异度、准确性、阳性预测值、阴性预测值分别为89.77%、50.00%、88.89%、98.75%、10.00%。腔内超声诊断肛周脓肿及瘘管形成的敏感度、特异度、准确性、阳性预测值、阴性预测值分别为88.64%、50.00%、87.78%、98.73%、10.00%。经会阴高频超声、腔内超声联合诊断肛周脓肿及瘘管形成的敏感度、准确性、阳性预测值分别为95.45%、93.33%、97.67%;联合检查的敏感度、准确性高于二者单独检测。结论:术前应用经会阴高频超声联合腔内超声评估肛周脓肿及瘘管形成的准确性较高,值得临床推广应用。
Objective To analyze the accuracy of preoperative evaluation of perianal abscess and fistula formation using transperineal high-frequency ultrasound combined with endoluminal ultrasound.Methods A total of 90patients with suspected perianal abscess and fistula formation in our hospital from October 2021to June 2024were selected as participants.All patients underwent transperineal high-frequency ultrasound and endoluminal ultrasound examinations.The pathological results were used as the gold standard to evaluate the detection and diagnostic value of transperineal high-frequency ultrasound,endoscopic ultrasound,and their combined diagnosis for perianal abscess and fistula formation.The detection situation of perianal abscess and fistula formation in different pathological types by transperineal high-frequency ultrasound,endoscopic ultrasound,and their combined detection was also evaluated.Results Among the 90 patients with suspected perianal abscess and fistula formation,88cases were diagnosed with perianal abscess and fistula by pathological examination;80cases were detected by transperineal high-frequency ultrasound;79cases were detected by endoluminal ultrasound;86cases were detected by the combined examination of transperineal high-frequency ultrasound and endoluminal ultrasound.Pathological examination showed 20cases of typeⅠ,21cases of typeⅡ,14cases of typeⅢ,15cases of typeⅣ,18cases of typeⅤ,and 2cases without lesions.The combined examination of transperineal high-frequency ultrasound and endoluminal ultrasound detected 21cases of typeⅠ,20cases of typeⅡ,15cases of typeⅢ,14cases of typeⅣ,16cases of typeⅤ,and 4cases without lesions.The sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of transperineal high-frequency ultrasound in diagnosing perianal abscess and fistula formation were 89.77%,50.00%,88.89%,98.75%,and 10.00%respectively.The sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of endoluminal ultrasound in diagnosing perianal abscess and fistula formation were 88.64%,50.00%,87.78%,98.73%,and 10.00%respectively.The sensitivity,accuracy,and positive predictive value of the combined diagnosis of transperineal high-frequency ultrasound and endoluminal ultrasound for perianal abscess and fistula formation were 95.45%,93.33%,and 97.67%respectively.The sensitivity and accuracy of the combined examination were higher than those of the individual examination.Conclusion Preoperative evaluation of perianal abscess and fistula formation using transperineal high-frequency ultrasound combined with endoluminal ultrasound has high accuracy and is worthy of clinical promotion and application.
