摘要
目的分析国家慢性病管理服务中老年慢性心力衰竭人群治疗依从性现状及影响因素,为提高患者治疗依从性提供指导依据。方法选择2020年8月-2024年7月简阳市中医医院收治的518例老年慢性心力衰竭患者为研究对象,在其参与国家基本公共卫生慢性病健康管理心力衰竭患者服务管理后的第3个月进行一般资料、慢性心力衰竭治疗依从性(Morisky服药依从性问卷)、慢性病患者健康素养调查量表、焦虑自评量表(SAS)和抑郁自评量表(SDS)调查,根据患者治疗依从性将其分为依从性良好组与依从性不佳组。采用多因素Logistic回归分析老年慢性心力衰竭人群国家慢性病管理服务治疗依从性的影响因素。结果纳入的患者中,治疗依从性良好者322例,依从良好率为65.71%。多因素Logistic回归分析显示,年龄、文化程度、同服药物种类、健康素养得分、SAS得分、SDS得分是老年慢性心力衰竭患者在国家慢性病管理服务中治疗依从性的影响因素(P<0.05)。结论老年慢性心力衰竭患者国家慢性病管理服务的治疗依从性还有待提升,而除了年龄、文化程度、同服药物种类等影响因素外,健康素养和心理状态也对其有较大影响。
Objective To analyze the status quo and influencing factors of treatment compliance of elderly population with chronic heart failure(CHF)in national chronic disease management service,and to provide guidance for enhancing the treatment compliance of patients.Methods Totally 518 elderly patients with CHF in Jianyang Chinese Medicine Hospital were selected from August 2020 to July 2024,and the general data,treatment compliance of chronic heart failure(Morisky Medication Adherence Questionnaire),health literacy survey scale for patients with chronic diseases,self-rating anxiety scale(SAS)and self-rating depression scale(SDS)were investigated at 3 months after patients participated in heart failure management service during national basic public health chronic disease health management.According to the treatment compliance of patients,they were classified into good compliance group and poor compliance group.Multivariate Logistic regression analysis was adopted to analyze the influencing factors of treatment compliance of national chronic disease management service in elderly CHF population.Results Among 490 enrolled patients,there were 322 cases with good treatment compliance,with a good compliance rate of 65.71%.Multivariate Logistic regression analysis suggested that age,education level,types of co-administered drugs,health literacy score,SAS score and SDS score were the influencing factors of treatment compliance of national chronic disease management service in elderly population with CHF(P<0.05).Conclusion The treatment compliance of national chro-nic disease management service in elderly CHF population needs to be improved,and the influencing factors include health literacy and psychological status in addition to age,education level and types of co-administered drugs.
