摘要
目的分析盐酸米诺环素与Vitapex糊剂根管充填联合治疗牙周牙髓联合病变的效果。方法非随机选取2023年1—12月菏泽医学专科学校附属医院收治的86例牙周牙髓联合病变患者为研究对象,按治疗方法分为两组。对照组(43例)采用Vitapex糊剂根管充填治疗,观察组(43例)采用盐酸米诺环素联合Vitapex糊剂根管充填治疗。比较两组临床疗效、炎性介质、牙龈指数、不良反应发生率。结果观察组总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,观察组白细胞介素6、肿瘤坏死因子α水平低于对照组,差异均有统计学意义(P均<0.05)。治疗后,观察组探诊深度(1.45±0.24)mm、牙龈出血指数(1.62±0.12)%、牙龈指数(0.91±0.14)分、菌斑指数(0.81±0.22)分,低于对照组的(2.34±0.41)mm、(2.34±0.24)%、(1.36±0.28)分、(1.31±0.42)分,差异均有统计学意义(t=12.284,17.595,9.426,6.915;P均<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论在牙周牙髓联合病变治疗中选择盐酸米诺环素联合Vitapex糊剂根管填充方案可改善牙龈沟液炎性反应及牙周指数,提升整体治疗效果,确保治疗及预后效果。
Objective To analyze the effect of minocycline hydrochloride combined with Vitapex paste root canal filling in the treatment of combined periodontal-endodontic lesions.Methods A total of eighty-six patients with combined periodontal-endodontic lesions admitted to the Affiliated Hospital of Heze Medical College from January to December 2023 were non-randomly selected and divided into two groups according to the treatment method.The control group(forty-three cases)was treated with Vitapex paste root canal filling,and the observation group(forty-three cases)was treated with minocycline hydrochloride combined with Vitapex paste root canal filling.The clinical efficacy,inflammatory mediators,gingival index,and incidence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of interleukin 6 and tumor necrosis factor-αin the observation group were lower than those in the control group,and the differences were statistically significant(all P<0.05).After treatment,the probing depth(1.45±0.24)mm,gingival bleeding index(1.62±0.12)%,gingival index 0.91±0.14,plaque index 0.81±0.22 in the observation group were lower than(2.34±0.41)mm,(2.34±0.24)%,1.36±0.28,1.31±0.42 in the control group,and the differences were statistically significant(t=12.284,17.595,9.426,6.915;all P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The combination of minocycline hydrochloride and Vitapex paste root canal filling in the treatment of combined periodontal-endodontic lesions can improve the inflammatory response in gingival crevicular fluid and periodontal indices,enhance the overall therapeutic effect,and ensure the treatment and prognosis.
