摘要
目的:探讨超声造影定量技术联合miR-374a-5p预测宫颈癌术后下肢深静脉血栓(LEDVT)效能。方法:选取2022年2月至2024年2月秦皇岛市第一医院收治的189例宫颈癌患者,均行宫颈癌根治术,根据术后2周内是否出现LEDVT分为LEDVT组(n=35)和N-LEDVT组(n=154)。统计两组一般资料、超声造影定量参数(dValue、TTP、K)、miR-374a-5p表达,Pearson分析超声造影定量参数与miR-374a-5p相关性,偏相关性分析超声造影定量参数、miR-374a-5p与LEDVT相关性,并通过受试者操作特征(ROC)曲线、净重分类改善指数(NRI)和综合判别改善指数(IDI)评估预测效能。结果:与N-LEDVT组相比,LEDVT组患者的活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)、纤维蛋白原(FIB)、凝血酶时间(TT)、miR-374a-5p表达水平及超声造影定量参数均显著升高(P<0.05);宫颈癌术后LEDVT患者dValue、TTP、K与miR-374a-5p呈正相关(P<0.05);miR-374a-5p、dValue、TTP、K与宫颈癌术后LEDVT发生呈正相关(P<0.05);联合miR-374a-5p与超声造影定量参数(dValue、TTP、K)的预测效能与凝血四项(APTT、PT、FIB、TT)相近,且优于单一指标的预测效能。结论:宫颈癌术后LEDVT患者miR-374a-5p表达升高,超声造影定量参数dValue、TTP、K也呈现高值,四者间呈显著正相关,联合检测有助于提高LEDVT预测效能,协助临床诊治。
Objective:To investigate the efficacy of quantitative technique of contrast-enhanced ultrasound technology combined with miR-374a-5p in predicting deep vein thrombosis(LEDVT)after cervical cancer surgery.Methods:A total of 189 cervical cancer patients admitted to the Qinhuangdao First Hospital from February 2022 to February 2024,all of whom underwent radical cervical cancer surgery,were classified into the LEDVT group(n=35)and the N-LEDVT group(n=154)according to whether they developed LEDVT within 2 weeks after surgery.The general data,quantitative parameters of contrast-enhanced ultrasound(dValue、TTP、K),and miR-374a-5p expression of the two groups were counted,and Pearson analysis was used to analyze the correlation between quantitative parameters of contrast-enhanced ultrasound and miR-374a-5p,and biased correlation was used to analyze the correlation between quantitative parameters of contrast-enhanced ultrasound,miR-374a-5p and LEDVT,and the predictive efficacy was assessed by receiver operating characteristic curve(ROC)and net reclassification improvement index(NRI)and integrated discrimination improvement index(IDI).Results:Compared with the N-LEDVT group,patients in the LEDVT group exhibited significantly elevated levels of activated partial thromboplastin time(APTT),prothrombin time(PT),fibrinogen(FIB),thrombin time(TT),miR-374a-5p expression,and quantitative parameters of contrast-enhanced ultrasound(P<0.05).Postoperative LEDVT patients with cervical cancer showed a positive correlation between dValue,TTP,K,and miR-374a-5p(P<0.05).miR-374a-5p,dValue,TTP,and K were positively correlated with the occurrence of postoperative LEDVT in cervical cancer patients(P<0.05).The predictive performance of combining miR-374a-5p with quantitative parameters of contrast-enhanced ultrasound(dValue,TTP,K)was similar to that of the four coagulation tests(APTT,PT,FIB,TT)and superior to that of a single indicator.Conclusion:In patients with LEDVT after cervical cancer surgery,the expression of miR-374a-5p is elevated,and the quantitative parameters of contrast-enhanced ultrasound,including dValue,TTP,and K,also exhibit high values.There is a significant positive correlation among the four parameters.Combined detection can help improve the predictive efficacy of LEDVT and assist clinical diagnosis and treatment.
