摘要
目的探讨应用内镜下冷圈套息肉切除术(CSP)治疗结直肠亚蒂型息肉患者的临床效果。方法选取2020年10月至2023年10月河南省胸科医院收治的结直肠亚蒂型息肉患者共计64例,以随机数字表法分成研究组(32例)与对照组(32例),给予研究组CSP治疗,给予对照组内镜下黏膜切除术治疗,比较两组围手术期指标、疼痛程度、应激反应指标、并发症。结果两组息肉完整切除率比较,差异无统计学意义(P>0.05),研究组息肉切除、首次排气、首次排便时间更短(P<0.05);两组术后2、3 d疼痛程度数字评分法(NRS)评分较术后1 d降低,术后3 d评分较术后2 d降低,研究组术后1、2、3 d NRS评分较对照组更低(P<0.05);术后1 d,两组皮质醇(Cor)、去甲肾上腺素(NE)水平升高,研究组较对照组更低(P<0.05);研究组并发症发生率[3.13%(1/32)]较对照组[25.00%(8/32)]更低(P<0.05)。结论结直肠亚蒂型息肉患者应用CSP治疗,能够改善围手术期指标,减轻术后疼痛,对应激反应指标水平影响较小,降低并发症发生率。
Objective To explore the clinical efficacy of endoscopic cold snare polypectomy(CSP)in the treatment of patients with sub-pedicle colorectal polyps.Methods A total of 64 patients with sub-pedicle colorectal polyps who were admitted in Henan Chest Hospital from October 2020 to October 2023 were selected,and randomly divided into the study group(32 cases,CSP treatment)and the control group(32 cases,endoscopic mucosal resection treatment).The perioperative indicators,pain level,stress response indicators,and complications in both groups were compared.Results In the complete polypectomy rate,both groups were no statistically significant differences(P>0.05).In comparison of the control group,and the polypectomy time,first exhaust time,and first defecation time were shorter(P<0.05).In comparison of 1 day after surgery,the NRS scores on 2,3 days after surgery in both groups were lower(P<0.05).In comparison of 2 days after surgery,the NRS scores on 3 days after surgery were lower(P<0.05).In comparison of the control group,the NRS scores at 1,2 and 3 days after surgery of the study group were lower(P<0.05).On the 1 day after surgery,the cortisol(Cor)and norepinephrine(NE)levels were increased in both groups,the study group was lower(P<0.05).In incidence of complications,in comparison of the control group 25.00%(8/32),the study group 3.13%(1/32)was lower(P<0.05).Conclusion CSP treatment can improve perioperative indicators,alleviate postoperative pain,the impact on the levels of stress response indicators is relatively small,and reduce the incidence of complications in patients with sub-pedicle colorectal polyps.
