摘要
目的探讨氨磺必利与奥氮平联合治疗对难治性精神分裂症的效果。方法按随机数字表法将2021年3月至2024年3月于抚州市第三医院治疗的70例难治性精神分裂症患者分为两组,每组35例。对照组采用奥氮平联合氯氮平治疗,观察组采用奥氮平联合氨磺必利治疗,两组均治疗2个月。比较两组临床疗效,精神症状,神经营养因子指标[胶质纤维酸性蛋白(GFAP)、神经生长因子(NGF)、同型半胱氨酸(Hcy)],生活质量[生活质量综合评定问卷(GQOLI-74)评分],认知功能(随机错误数、持续错误数、完成分类数、正确分类数)及不良反应。结果相比于对照组,观察组治疗总有效率较高(P<0.05);观察组治疗后阳性和阴性症状量表(PANSS)评分较低(P<0.05);观察组治疗后随机错误数、持续错误数较低,完成分类数、正确分类数较高,GFAP及Hcy水平较低,NGF水平较高(P<0.05);观察组治疗后GQOLI-74评分较高(P<0.05)。两组不良反应比较,差异无统计学意义(P>0.05)。结论奥氮平联合氨磺必利治疗难治性精神分裂症可更有效改善神经营养因子指标,提高认知功能及生活质量,缓解患者精神症状,疗效确切,安全可靠,具有广泛应用前景。
Objective To investigate the effect of amisulpride combined with olanzapine in the treatment of refractory schizophrenia.Methods 70 patients with refractory schizophrenia treated in The Third Hospital of Fuzhou from March 2021 to March 2024 were divided into two groups with 35 cases in each group by random number table method.The control group was treated with olanzapine combined with clozapine,and the observation group was treated with amisulpride combined with olanzapine,both groups were treated for 2 months.The clinical efficacy,mental symptoms,neurotrophic factors(GFAP,NGF,Hcy),quality of life(GQOLI-74 scores),cognitive function(random errors,persistent errors,completed classification,correct classification)and adverse reactions were compared between the two groups.Results Compared with the control group,the total effective rate of the observation group was higher(P<0.05);the positive and negative symptom scale(PANSS)scores in the observation group were lower after treatment(P<0.05);the number of random errors and persistent errors after treatment in the observation group was lower,the number of completed classification and correct classification was higher,the levels of GFAP and Hcy were lower,and the level of NGF was higher(P<0.05);the quality of life scores of the observation group were higher after treatment(P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion Amisulpride combined with olanzapine can effectively improve neurotrophic factors,improve cognitive function and quality of life,and alleviate mental symptoms of patients with refractory schizophrenia.It is effective,safe and reliable,and has a wide application prospect.
