摘要
目的探讨医院-社区-家庭三元联动延续护理模式对下肢静脉性溃疡患者自我管理能力、自我效能感、溃疡治愈率、医院与社区会诊率相关指标的影响。方法选取2022年8月至2023年12月某社区医院123例下肢静脉性溃疡患者作为研究对象,按随机数字表法将患者分为观察组(61例)和对照组(62例),对照组采用传统的社区伤口护理模式,观察组在此基础上实施基于医院-社区-家庭三元联动延续护理模式干预,进行为期6个月的干预。分别于干预前及干预后3个月、6个月评估患者自我管理能力、自我效能感,比较干预后6个月两组患者的溃疡治愈率、医院与社区会诊率。结果实施医院-社区-家庭三元联动延续护理模式后3个月、6个月观察组下肢静脉性溃疡患者自我管理能力和自我效能感均高于对照组(P<0.05),干预6个月后观察组溃疡治愈率高于对照组(P<0.05),医院与社区会诊率低于对照组(P<0.05)。结论医院-社区-家庭三元联动延续护理模式能有效提高下肢静脉性溃疡患者自我管理能力、自我效能感和治愈率,降低了医院与社区会诊率,对提高患者治疗效果具有重要意义。
Objective To explore the impact of the continuous nursing model based on the tripartite linkage of hospital-community-family on the relevant indicators in patients with lower extremity venous ulcers,including the self-management ability,self-efficacy,ulcer cure rate,and the consultation rate between hospitals and communities.Methods A total of 123 patients with lower extremity venous ulcers in a certain community hospital from August 2022 to December 2023 were selected as the research objects.The patients were divided into the observation group(61 cases)and the control group(62 cases)according to the random number table method.The patients in the control group adopted the traditional community wound care,while the patients in the observation group received the continuous nursing model based on the tripartite linkage of hospital-community-family.The patients'self-management behaviors and self-efficacy were evaluated before the intervention and 3 months and 6 months after the intervention.The ulcer cure rate and the consultation rate between hospitals and communities were compared 6 months after the intervention.Results Three months and 6 months after the implementation of the continuous nursing model,the self-management ability and the self-efficacy of patients in the observation group were higher than those in the control group(P<0.05).After 6 months of intervention,the ulcer cure rate of patients in the observation group was higher than that of the control group(P<0.05),and the consultation rate between the hospital and the community was lower than that of the control group(P<0.05).Conclusion The continuous nursing model based on the tripartite linkage of hospital-community-family can effectively improve patients'self-management ability,self-efficacy,and the cure rate and reduce the consultation rate between hospitals and communities.It is of great significance for improving the treatment effect of patients with lower limb venous ulcers.
