摘要
目的探讨麝香保心丸辅助治疗对心力衰竭合并房颤患者心功能及血清心衰标志物的影响。方法将兴国县中医院2023年8月-2025年3月收治的68例老年心力衰竭合并房颤患者按随机数字表法分为观察组34例和对照组34例。所有患者均行常规基础治疗,对照组患者在此基础上予以口服盐酸胺碘酮片,观察组患者则在常规基础治疗上加服麝香保心丸治疗。比较两组患者临床治疗效果、血清心衰标志物水平[脑钠肽(BNP)、肌肌钙蛋白Ⅰ(CTnⅠ)]、心功能[N端脑钠肽前体(NT-proBNP)、QT离散度(QTd)、QT间期最小值(QTmin)、最大值(QTmax)、射血分数(LVEF)、左心室舒张末期内径(LVDD)]、不良反应发生情况[恶心呕吐、窦性心动过缓、谷丙转氨酶(ALT)水平上升]。结果与对照组总有效率的70.59%相比,观察组治疗总有效率为97.06%,更高(P<0.05)。两组治疗后BNP、cTnⅠ水平均低于治疗前,且观察组低于对照组(P<0.05)。与治疗前相比,两组治疗后NT-proBNP、QTd、QTmin、QTmax、LVDD水平均降低,而LVEF水平升高,且与对照组相比,观察组NT-proBNP、QTd、QTmin、QTmax、LVDD水平更低,而LVEF水平更高(P<0.05)。观察组不良反应发生率低于对照组(P<0.05)。结论麝香保心丸作为辅助用药可有效增强心衰伴房颤患者的治疗效果,优化其心功能状态,同时下调心衰标志物水平。该治疗方案不良反应发生率较低,在临床实践中具有突出的应用优势。
Objective To investigate the effect of Shexiang Baoxin pills as an adjuvant therapy on cardiac function and serum heart failure markers in patients with heart failure and atrial fibrillation.Methods Sixty-eight elderly patients with heart failure and atrial fibrillation admitted to Xingguo County Hospital of Traditional Chinese Medicine from August 2023 to March 2025 were divided into observation group(34 patients)and control group(34 patients)by using a random number table method.All patients were treated with routine basic treatment,on this basis,the control group was given oral amiodarone hydrochloride tablets,while the observation group was treated with Shexiang Baoxin pill on the basis of routine basic treatment.The clinical efficacy,serum heart failure marker levels[brain natriuretic peptide(BNP),troponinⅠ(CTnⅠ)],cardiac function[N-terminal pro-brain natriuretic peptide(NT-proBNP),QT dispersion(QTd),QT interval minimum(QTmin),maximum(QTmax),ejection fraction(LVEF),Left ventricular end-diastolic diameter(LVDD)],adverse reactions[nausea and vomiting,sinus bradycardia,elevated alanine aminotransferase(ALT)level]were compared between the two groups.Results Compared with 70.59%of the total effective rate of the control group,the total effective rate of the observation group was 97.06%,which was higher(P<0.05).Compared with before treatment,the levels of NT-proBNP,QTd,QTmin,QTmax and LVDD in the two groups were decreased after treatment,while the level of LVEF was increased;compared with the control group,the levels of NT-proBNP,QTd,QTmin,QTmax and LVDD in the observation group were lower,while the level of LVEF was higher(P<0.05).The observation group showed lower incidence of adverse reactions than that in the control group(P<0.05).Conclusion As an adjuvant drug,Shexiang Baoxin pills can effectively enhance the therapeutic effect of patients with heart failure and atrial fibrillation,optimize their cardiac function,and down-regulate the level of heart failure markers.Meanwhile,the incidence of adverse reactions of this treatment is low,and it has outstanding application advantages in clinical practice.
