摘要
慢性前列腺炎(Chronic prostatitis,CP)是泌尿男科的常见病,以盆腔区域疼痛或不适、排尿异常等症状为特征,多发于50岁以下的成年男性。病情多缠绵难愈,长期的疼痛刺激以及异常的排尿症状,对患者的生活质量和身心健康影响较大。目前,CP的发病机制尚未明确,是多途径、多因素共同作用的结果,临床表现复杂多样,病情易反复,成为困扰临床工作者的难题。2009年,Shoskes DA根据患者临床症状,提出并制定了UPOINT临床表型分型系统,将临床表现分为6种阳性域,为前列腺炎患者进行个体化精准治疗提供了有效的工具。中医经过长期实践,将CP病机归纳为肾虚、血瘀、湿热三方面,根据患者证候特点辨证施治。二者均注重个体化差异,主张将复杂的临床表现进行分类归纳,根据分型结果及病机特点进行治疗。鉴于此,文中旨在探讨在临床上如何将UPOINT分型系统与中医辨证施治相结合,形成中西医结合的个体化治疗方案,从而更好地发挥二者的优势,进一步提高CP临床疗效和患者生活质量。
Chronic prostatitis(CP)is a common disease in urology,characterized by pelvic pain or discomfort,abnormal urination,and other symptoms,and occurs mostly in adult males under the age of 50.The disease is often persistent and difficult to cure.Long-term pain and abnormal urination greatly impact patients'quality of life and physical and mental health.At present,the pathogenesis of CP remains unclear,which is a result of multiple pathways and actors.Its clinical manifestations are complex and diverse,and the condition tends to recur,posing a challenge for clinical workers.In 2009,Shoskes DA proposed and developed the UPOINT system based on patients'clinical symptoms,dividing clinical manifestations into six positive domains,thereby providing an effective tool for individualized precision treatment of prostatitis.Through long-term practice,TCM has attributed the pathogenesis of CP to kidney deficiency,blood stasis,and damp-heat,and differential diagnosis and treatment are applied according to the characteristics of patients'syndromes.Both approaches emphasize individual differences,advocate the classification of complex clinical manifestations,and provide treatment strategies based on classification and pathogenesis.In view of this,this paper aims to explore how to integrate the UPOINT system with TCM's differential diagnosis and treatment in clinical practice,so as to develop individualized treatment plans combining TCM and Western medicine.This integrated approach may better leverage the advantages of both and further improve the clinical efficacy and quality of life for patients with chronic prostatitis.
