摘要
目的:探究邵氏化瘀软坚固气汤加减联合针刺对慢性前列腺炎患者前列腺炎症、前列腺体积和尿流动力学的影响。方法:将2019年10月~2020年10月期间于我院就诊的84例慢性前列腺炎患者随机分为A组、B组、C组各28例,C组采用盐酸坦洛新,B组在C组基础上予以针刺治疗,A组在B组基础上予以邵氏化瘀软坚固气汤加减治疗,比较治疗6w后三组中医证候疗效差异,比较治疗前及治疗6w后三组中医症状积分、炎性因子[肿瘤坏死因子-α(TNF-α)、干扰素-γ(IFN-γ)、白细胞介素-12(IL-12)]、前列腺体积(前列腺体体积、前列腺移行区体积)、前列腺病变程度[美国国立卫生院慢性前列腺炎症状指数(NIH-CPSI)评分]、尿流动力学[平均尿流率(AFR)、最大尿流率(MFR)、膀胱残余尿量(PVR)]变化。结果:治疗6w后,A组、B组总有效率均显著高于C组(P<0.05),A组、B组总有效率差异不显著(P>0.05);三组中医症状积分、TNF-α、IFN-γ、IL-12、前列腺体体积、前列腺移行区体积、PVR水平均较治疗前显著降低,且A组明显低于同一时间B组、C组(P<0.05),B组明显低于同一时间C组(P<0.05);三组NIH-CPSI评分、AFR、MFR水平均较治疗前显著升高,且A组明显高于同一时间B组、C组(P<0.05),B组明显高于同一时间C组(P<0.05)。结论:邵氏化瘀软坚固气汤加减联合针刺治疗慢性前列腺炎效果显著,可有效抑制患者炎症反应,促进前列腺体积恢复,改善其尿流动力学,临床推广意义较高。
Objective:To explore the effect of Modified SHAO’s Huayu Ruanjian Guqi Decoction combined with acupuncture on prostate inflammation,prostate volume and urodynamics for patients with chronic prostatitis.Methods:Totally 84 patients with chronic prostatitis who were treated in our hospital from October 2019 to October 2020 were randomly divided into a group A,a group B and a group C,with 28 cases in each group.The group C was treated with Tamsulosin Hydrochloride,and the group B was combined with acupuncture on the basis of the group C,and the group A was added with Modified SHAO’s Huayu Ruanjian Guqi Decoction on the basis of the group B.The difference in efficacy of TCM syndromes in the three groups was compared after 6 weeks of treatment,and the scores of TCM symptoms,inflammatory factors[tumor necrosis factor-α(TNF-α),interferon-γ(IFN-γ),interleukin-12(IL-12)],prostate volume(prostate body volume,prostate transition zone volume),degree of prostate lesions[National Institutes of Health Chronic Prostatitis Symptom Index(NIH-CPSI)score]and urodynamics[average urine flow rate(AFR),maximum urine flow rate(MFR),post-void residual volume(PVR)]were compared among the three groups before treatment and after 6 weeks of treatment.Results:After 6 weeks of treatment,the total effective rate of the group A and the group B was significantly higher than that of the group C(P<0.05),and the total effective rate of the group A was not significantly different form the group B(P>0.05).The scores of TCM symptoms,levels of TNF-α,IFN-γand IL-12,prostate body volume,prostate transition zone volume and PVR in the three groups were significantly decreased compared to before treatment,and the above indexes of the group A were significantly lower than those of the group B and the group C at the same time point(P<0.05),and the above indexes of the group B were significantly lower than those of the group C(P<0.05).The score of NIH-CPSI and levels of AFR and MFR in the three groups were significantly increased compared with those before treatment,and the score and levels of the group A were significantly higher than those of the group B and the group C(P<0.05),and the score and levels of the group B were significantly higher than those of the group C at the same time point(P<0.05).Conclusion:Modified SHAO’s Huayu Ruanjian Guqi Decoction combined with acupuncture has a significant effect in the treatment of chronic prostatitis.And it can effectively inhibit the inflammatory response,promote the recovery of prostate volume,and improve the urodynamics,with a high clinical promotion significance.
