摘要
目的:比较热牙胶垂直加压法与冷牙胶侧方加压法根管充填治疗根尖周病患者的效果。方法:选取2022年9月至2023年9月该院收治的84例根尖周病患者进行前瞻性研究,按照随机数字表法将其分为研究组与对照组各42例,两组均进行根管充填,对照组采用冷牙胶侧方加压法,观察组采用热牙胶垂直加压法,比较两组临床疗效,根管充填情况,治疗前后牙周指标[龈沟出血指数(SBI)、牙周袋深度(PD)、牙龈指数(GI)、菌斑指数(PLI)]水平、咀嚼功能(咀嚼效率、咬合力),以及不良事件发生率。结果:研究组治疗总有效率为88.10%,高于对照组的69.05%,差异有统计学意义(P<0.05);研究组根管恰填率高于对照组,根管充填时间短于对照组,差异有统计学意义(P<0.05);治疗后1个月,两组SBI、PD、GI、PLI水平均低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05);治疗后3个月,两组咀嚼效率、咬合力均高于治疗前,且研究组高于对照组,差异有统计学意义(P<0.05);两组不良事件发生率比较,差异无统计学意义(P>0.05)。结论:热牙胶垂直加压法根管充填治疗根尖周病患者可提高根管充填效果与咀嚼功能,改善牙周指标水平,缩短根管充填时间的效果优于冷牙胶侧方加压法根管充填。
Objective:To compare effects of hot gutta-percha vertical condensation and cold gutta-percha lateral condensation root canal filling in treatment of periapical disease.Methods:A prospective study was conducted on 84 patients with periapical disease admitted to this hospital from September 2022 to September 2023.According to the random number table method,they were divided into study group and control group,42 cases in each group.Both groups were given root canal filling.The control group was treated with cold gutta-percha lateral condensation,while the observation group was treated with hot gutta-percha vertical condensation.The clinical efficacy,the root canal filling conditions,the periodontal indexes[sulcus bleeding index(SBI),periodontal pocket depth(PD),gingival index(GI),plaque index(PLI)]levels,the masticatory function(masticatory efficiency and bite force)before and after the treatment,and the incidence of adverse events were compared between the two groups.Results:The total effective rate of treatment in the study group was 88.10%,which was higher than 69.05%in the control group,and the difference was statistically significant(P<0.05).The root canal filling rate of the study group was higher than that of the control group,the root canal filling time was shorter than that of the control group,and the differences were statistically significant(P<0.05).One month after the treatment,the levels of SBI,PD,GI and PLI in the two groups were lower than those before the treatment,those in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).Three months after the treatment,the masticatory efficiency and the bite force of the two groups were higher than those before the treatment,those of the study group were higher than those of the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse events between the two groups(P>0.05).Conclusions:Hot gutta-percha vertical condensation root canal filling in the treatment of periapical periodontitis can improve the root canal filling effects and the masticatory function,improve the periodontal index levels,and shorten the root canal filling time.Moreover,it is superior to cold gutta-percha lateral condensation root canal filling.
