EB病毒感染并发肺炎支原体肺炎患儿的血清代谢组学及临床免疫特征分析

Analysis of serum metabolomics and clinical immune characteristics in children with epstein-barr virus infection complicated with mycoplasma pneumoniae pneumonia

摘要 目的探讨EB病毒(EBV)感染并发肺炎支原体(MP)肺炎患儿的血清代谢组学及临床免疫特征变化。方法选取2022年12月~2024年12月庆阳市人民医院收治的124例EBV感染患儿为病例组,且根据是否并发MP肺炎分为并发组(n=16)、未并发组(n=108);另选取同期62例健康体检儿童为对照组,比较并发组、未并发组和对照组的临床资料、血清代谢组学、淋巴细胞、免疫球蛋白(Ig)、炎症标志物水平。采用多因素Logistic回归模型分析影响EBV并发MP肺炎的独立危险因素。结果并发组抗病毒药物使用时间和发热时间长于未并发组,EBV载量、淋巴结肿大和肝脾肿大占比高于未并发组,年龄低于未并发组(P<0.05);并发组、未并发组和对照组亮氨酸、亚油酸、苯丙氨酸、尿素、二十碳五烯酸、乳酸、CD8^(+)、IgM、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、γ干扰素(IFN-γ)、白细胞介素-10(IL-10)、红细胞沉降率(ESR)水平比较,并发组>未并发组>对照组(P<0.05);三组IgG、CD3^(+)、CD4^(+)水平比较,差异无统计学意义(P>0.05);多因素Logistic回归分析结果显示,亚油酸、苯丙氨酸、尿素、CD8^(+)、IL-6、IgM、IL-10水平升高是EBV并发MP肺炎的独立危险因素(P<0.05)。结论EBV感染并发MP肺炎患儿血清中的亚油酸、苯丙氨酸、尿素及CD8^(+)、IL-6、IgM、IL-10水平升高,提示上述指标具有作为生物标志物及干预靶点的潜在临床价值。 Objective To investigate the changes of serum metabolomics and clinical immune characteristics in children with epstein-barr virus(EBV)infection complicated with Mycoplasma pneumoniae(MP)pneumonia.Methods A total of 124 children with EBV infection admitted to Qingyang People's Hospital from December 2022 to December 2024 were selected as the case group,and were divided into the concurrent group(n=16)and the non-concurrent group(n=108)according to whether they were complicated with MP pneumonia.Another 62 healthy children who underwent physical examinations during the same period were selected as the control group.The clinical data,serum metabolomics,lymphocytes,immunoglobulin(Ig),and inflammatory marker levels of the concurrent group,the non-concurrent group,and the control group were compared.Multivariate Logistic regression model was used to analyze the independent risk factors influencing EBV complicated with MP pneumonia.Results The duration of antiviral drug use and fever duration in the concurrent group were longer than those in the non-concurrent group.The EBV load,the proportions of lymph node enlargement and hepatosplenomegaly in the concurrent group were higher than those in the non-concurrent group,and the age was lower than that in the non-concurrent group(P<0.05).Comparison of leucine,linoleic acid,phenylalanine,urea,eicosapentaenoic acid,lactic acid,CD8^(+),IgM,tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interferon-γ(IFN-γ),interleukin-10(IL-10),and erythrocyte sedimentation rate(ESR)levels among the concurrent group,the non-concurrent group and the control group,concurrent group>non-concurrent group>control group(P<0.05);There was no statistically significant difference in the levels of IgG,CD3^(+)and CD4^(+)among the three groups(P>0.05).The results of multivariate Logistic regression analysis showed that elevated levels of linoleic acid,phenylalanine,urea,CD8^(+),IL-6,IgM,and IL-10 were independent risk factors for EBV complicated with MP pneumonia(P<0.05).Conclusion The levels of linoleic acid,phenylalanine,urea,CD8^(+),IL-6,IgM and IL-10 in serum of children with EBV infection complicated with MP pneumonia increased,suggesting that the above indicators have potential clinical value as biomarkers and intervention targets.
