摘要
目的:探究超声引导下甲状腺细针穿刺检查(US-FNA)在甲状腺肿瘤定性诊断中的应用效果。方法:选取2022年1月—2023年12月于广州中医药大学顺德医院(佛山市顺德区中医院)就诊的250例甲状腺肿瘤患者作为研究对象,收集所有患者的临床资料。分析常规超声、US-FNA与病理诊断甲状腺肿瘤良恶性结果的一致性。以病理诊断结果为“金标准”,比较常规超声、US-FNA诊断甲状腺肿瘤良恶性的效能。结果:常规超声与病理诊断结果的一致性尚可(Kappa=0.544,P<0.001);US-FNA与病理诊断结果的一致性较好(Kappa=0.827,P<0.001)。US-FNA诊断甲状腺肿瘤良恶性的灵敏度、准确度高于常规超声(P<0.05);常规超声与US-FNA诊断甲状腺肿瘤良恶性的特异度比较,差异无统计学意义(P>0.05)。结论:US-FNA诊断甲状腺肿瘤性质的效果良好,与病理诊断的一致性较好,灵敏度、准确度较高。
Objective:To explore the application effect of ultrasound-guided fine-needle aspiration of the thyroid(US-FNA)in the qualitative diagnosis of thyroid tumors.Methods:A total of two hundred and fifty patients with thyroid tumors who visited Shunde Hospital of Guangzhou University of Chinese Medicine(Foshan Shunde District Hospital of Traditional Chinese Medicine)from January 2022 to December 2023 were selected as the research subjects,and the clinical data of all patients were collected.The consistency of the results of conventional ultrasound,US-FNA and pathological diagnosis of benign and malignant thyroid tumors were analyzed.Taking the pathological diagnosis results as the"gold standard",the efficacy of conventional ultrasound and US-FNA in diagnosing benign and malignant thyroid tumors was compared.Results:The consistency between the results of conventional ultrasound and pathological diagnosis was acceptable(Kappa=0.544,P<0.001).The consistency between US-FNA and the pathological diagnosis results was good(Kappa=0.827,P<0.001).The sensitivity and accuracy of US-FNA in diagnosing benign and malignant thyroid tumors were higher than those of conventional ultrasound(P<0.05).There was no statistically significant difference in the specificity of conventional ultrasound and US-FNA in diagnosing benign and malignant thyroid tumors(P>0.05).Conclusion:The effect of US-FNA in diagnosing the nature of thyroid tumors is good,with good consistency with pathological diagnosis,and high sensitivity and accuracy.
