摘要
目的:探讨前纵隔肿瘤行剑突下入路胸腔镜手术患者预后的相关影响因素.方法:前瞻性选取2019年2月至2024年3月期间在本院行剑突下入路胸腔镜手术治疗的60例前纵隔肿瘤患者作为研究对象.根据患者行剑突下入路胸腔镜手术3 m后的预后情况,将患者分为预后不良组和预后良好组.收集并比较两组的基线资料.分析前纵隔肿瘤患者行剑突下入路胸腔镜手术后预后的影响因素.结果:60例前纵隔肿瘤患者行剑突下入路胸腔镜手术治疗后3 m,发生并发症者9例,发生率为15.00%.预后不良组年龄≥60岁占比、糖尿病占比、肿瘤性质为恶性占比、肿瘤直径均高于预后良好组(P<0.05).多因素logistic回归分析结果显示,年龄≥60岁、糖尿病、肿瘤性质为恶性、肿瘤直径增大均是前纵隔肿瘤患者行剑突下入路胸腔镜手术后预后的影响因素(P<0.05).结论:年龄、糖尿病、肿瘤性质为恶性、肿瘤直径增大均是前纵隔肿瘤患者行剑突下入路胸腔镜手术后预后的影响因素.
Objective:To investigate the prognostic factors of patients undergoing thoracoscopic subxiphoid approach for anterior mediastinal tumors.Methods:Sixty patients with anterior mediastinal tumors who underwent thoracoscopic surgery with subxiphoid approach in our hospital from February 2019 to March 2024 were selected as the study objects.According to the prognosis after 3 months of thoracoscopic surgery with subxiphoid approach,the patients were divided into poor prognosis group and good prognosis group.Baseline data were collected and compared between the two groups.The factors influencing the prognosis of patients with anterior mediastinal tumor after thoracoscopic subxiphoid approach were analyazed.Results:In 60 patients with anterior mediastinal tumor treated with subxiphoid thoracoscopic approach 3 months later,complications occurred in 9 cases(15.00%).The proportion of age≥60 years old,the proportion of diabetes mellitus,the proportion of malignant tumor and the tumor diameter in the poor prognosis group were higher than those in the good prognosis group(P<0.05).Multivariate logistic regression analysis showed that age≥60 years old,diabetes mellitus,malignant tumor nature,and increase in tumor diameter were all prognostic factors for anterior mediastinal tumor patients undergoing thoracoscopic subxiphoid approach(P<0.05).Conclusion:Age,diabetes mellitus,malignancy of tumor and enlargement of tumor diameter are the prognostic factors of patients with anterior mediastinal tumor undergoing thoracoscopic subxiphoid approach.
