摘要
目的比较重组人粒细胞集落刺激因子(rhG-CSF)与聚乙二醇化rhG-CSF(PEG-rhG-CSF)对直肠癌化疗后中性粒细胞减少症的预防效果。方法将92例直肠癌化疗患者随机均分为PEG-rhG-CSF组和rhG-CSF组。rhG-CSF组于化疗后予以rhG-CSF皮下注射,PEG-rhG-CSF组于化疗后予以PEG-rhG-CSF皮下注射。比较两组Ⅲ、Ⅳ度中性粒细胞减少症发生率、发热性中性粒细胞减少症发生率、抗生素使用率、外周血中性粒细胞绝对值(ANC)变化及不良反应发生情况。结果PEG-rhG-CSF组Ⅲ度及Ⅲ、Ⅳ度中性粒细胞减少症总发生率、发热性中性粒细胞减少症发生率及抗生素使用率均低于rhG-CSF组(P<0.05),ANC恢复时间短于rhG-CSF组(P<0.05)。PEG-rhG-CSF组用药后第1、3、5天ANC均高于rhG-CSF组(P<0.05)。PEG-rhG-CSF组和rhG-CSF组用药期间不良反应发生率比较,差异无统计学意义(P>0.05)。结论相较于rhG-CSF,直肠癌化疗后使用PEG-rhG-CSF预防中性粒细胞减少症效果更优,能够更有效降低Ⅲ度及以上中性粒细胞减少症发生率,缩短ANC恢复时间,减少发热性中性粒细胞减少症的发生率以及抗生素的使用率,且两者安全性差异无显著性。
Aim To compare the efficacy of recombinant human granulocyte colony-stimulating factor(rhG-CSF)and pegylated rhG-CSF(PEG-rhG-CSF)in preventing neutropenia after chemotherapy for rectal cancer.Methods A total of 92 rectal cancer patients undergoing chemotherapy were randomly and equally divided into the PEG-rhG-CSF group and the rhG-CSF group.The rhG-CSF group received subcutaneous injections of rhG-CSF after chemotherapy,while the PEG-rhG-CSF group received subcutaneous injections of PEG-rhG-CSF after chemotherapy.The incidence rates of gradeⅢ,Ⅳneutropenia,febrile neutropenia,antibiotic usage rates,changes in absolute neutrophil count(ANC),and the occurrence of adverse reactions were compared between the two groups.Results The PEG-rhG-CSF group showed lower incidence rates of gradeⅢneutropenia,combined gradeⅢandⅣneutropenia,febrile neutropenia,and antibiotic usage rates compared to the rhG-CSF group(P<0.05),along with shorter ANC recovery time(P<0.05).On days 1,3,and 5 post-administration,ANC levels were significantly higher in the PEG-rhG-CSF group than those in the rhG-CSF group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between between the PEG-rhG-CSF group and the rhG-CSF group during medication(P>0.05).Conclusion Compared with rhG-CSF,the use of PEG-rhG-CSF after chemotherapy for rectal cancer demonstrates superior efficacy in preventing neutropenia.It more effectively reduces the incidence of gradeⅢor higher neutropenia,shortens the recovery time of ANC,decreases the occurrence of febrile neutropenia and antibiotic usage rates,with no significant difference in safety between the two treatments.
